My B&T GHM9 had its extractor blown out! Double charged round? Barrel obstruction? Out of battery?

In this video I talk about a major failure I had with one of my B&T firearms during a test of an aftermarket magazine. I have never had anything like this happen before. What do you all think? Is this something that can happen with double charges, barrel obstructions or out of battery firings?
