Monarchy 101: go round door to door with a sword, when the person answers say "by almight grace of god do you accept X to be you King and grant thee said title and subjugate yourself by here and almighty", if the person said no they would kill him and move onto the next door. after a while people will say yes and accept X as your king. It exists all in your heads, the more heads its in, the more it becomes real and important
