Insulin is a peptide hormone synthesized by the beta cells of the pancreas. It has the function to facilitate the entry of glucose inside the cells to maintain normal blood glucose levels. An increased level of insulin causes insulin resistance in which cells have an impaired sensitivity to insulin causing the glucose to stay in the blood. There is increasing evidence that insulin affects the regulation of nutrient homeostasis, cognition, memory, and neuroprotection in the brain. Is insulin resistance involved in the pathogenesis of Alzheimer's disease? Definitely, in the brain, insulin resistance alongside hyperinsulinaemia promotes the formation of advanced glycation end-products, which are compounds formed by the glycation of lipids and proteins. These compounds have been suggested to increase the expression of amyloid precursor protein and amyloid beta via reactive oxygen species. Amyloid precursor protein refers to the protein from which amyloid beta plaques originate. Amyloid beta aggregation results in inflammatory response leading to the release of pro inflammatory mediators including complement factors, chemokines and eicosanoids. These pro inflammatory mediators impair microglial clearance of beta amyloid plaques and increase microglia-mediated neuronal death and damage.
