The bit of dialog from the landlady has the accent written into the text. I'm not trying to be mocking or obnoxious or obscene about it, just reading it as it was written.

Ulster, in this context, is a type of overcoat.

Galen was a Greek physician of the 2nd century AD. I assume the use of the plural "Galens" here is meant to refer to physicians more generally, although such usage is definitely not anything you'd hear today.
