Today we'll be discussing pathologies that can result in a total hip replacement or total hip arthroplasty, and a summary of the surgery. A total hip replacement is most commonly performed due to osteoarthritis of the hip with age and use. The articular cartilage of the hip joint can de generate the articular cartilage, becomes frayed and rough, which exposes the bony surfaces beneath and makes movement. A previous injury, bony abnormalities of the femur or acetabulum, repetitive stress from physical activities and weakness of the hip stabilizers resulting in increased compression through the hip can all increase the likelihood of an osteoarthritic condition. Other risk factors such as obesity and a genetic disposition to arthritis may also play a role in hip degeneration. Severe osteoarthritis is by far the most common indication for performing a total hip. While less common rheumatoid arthritis and autoimmune disease in which the synovial membrane surrounding the hip is inflamed, [00:01:00] can lead to a total hip replacement, a vascular necrosis, a condition in which blood supply to the femoral head is minimized from a traumatic injury, fracture or disease can cause the femoral head to decay and warrant a total hip replacement. Sometimes a childhood disease can result in abnormal hip development that leads to an early onset of hip degeneration that requires a total hip replace. Symptoms that indicate a need for total hip replacement include hip stiffness, pain, and poor functional mobility that is very detrimental to a person's quality of life and is not responsive to conservative approaches like physical therapy, medications, or injections. A total hip replacement involves both components of the ball and socket joint. The degenerated femoral head is removed. A metal stem is then inserted into the hollow middle of the femoral. Attached to the stem is a prosthesis made of biocompatible metal or ceramic that replicates the ball component of the femoral head. Next, the damaged articular cartilage of the acetabulum is removed and replaced with a metal socket that is screwed or cemented in place. Finally, a [00:02:00] plastic, ceramic, or metal liner is inserted between the new ball and socket to improve joint congruency and provide a smooth gliding surface. I'm going to walk you through the biomechanics and functional movement of the hip joint femoral acetabular anatomy allows for three degrees of freedom, sagittal plane flexion, and extension, frontal plane, abduction, abduction, and transverse plane, internal and external rotation. In the sagittal plane, the hip can usually flex to 140 degrees. This is crucial for the mid-stance and swing through portions. As well as for squatting, the hip should extend to 15 degrees. This is needed for the push off and terminal hip extension phases of gait. In the frontal plane, the hip should be able to abduct to about 45 degrees and adduct to about 30 degrees. Maintaining adequate hip abduction during single leg stance and avoiding excessive collapse into hip abduction is crucial for healthy hips and. Poor control of single leg stand stability results in increased compressive forces through the weight-bearing portions of the hip and knee, which can result in a [00:01:00] generation over time in the transverse plane, the hip should be able to internally and externally rotate about 45 degrees in each direction. Adequate hip transverse plane motion is essential for reciprocal motions in the body, such as walking, running, and rotational sports like baseball, golf, and tennis. Today I'll be talking to you about meniscus injuries. Meniscus tears can occur as a result of an acute injury or a degenerative process associated with aging. The most common acute injuries involve either a twisting injury and or rapid change in direction with a foot planted on the ground or direct contact from an external force. Meniscus injuries may be classified in several different ways. One way to classify a meniscus tear is based on the severity of tear and includes qualifiers such as, Moderate or severe. Alternatively, classification can be based on the location of the tear. Peripheral tears are associated with a rich vascularity and a good healing potential. Central tears are associated with a poor vascularity and healing potential. Finally, meniscus tears can [00:01:00] also be classified based on their orientation. Examples are vertical, radial, horizontal o. Or complex, which is a combination of previously mentioned types of tears. The associated symptoms of a meniscus injury vary depending on the severity, location, and orientation of the injury, as well as the presence of any associated intraarticular problems. A common symptom presentation may include pain, which is aggravated during weightbearing activities, as well as swelling that may lead to knee stiffness and loss of motion with bending or terminal extens. In addition, during more severe meniscus tears, patients may experience catching, locking, or giving way. Injury to the meniscus leads to a disruption of anatomy and its inherent function. It consists of a massive muscular heart and a system of closed blood vessels comprising arteries. Arterials capillaries is veles and veins, and hence is also known as the cardiovascular. It acts as the connecting link between the neuromuscular system and organs, such as the digestive, respiratory, excretory, organs, et cetera. The heart transports blood to the tissues to maintain their normal functioning. It also helps to coordinate the activities of various organs by transporting the hormones to the target organ. There are two circuits of circulation, and both these circuits start from the heart and terminate in the heart. The blood is kept under circulation by two pumps of the heart, the right sided consisting of the right atrium and right ventricle, and the left sided consisting of the left atrium and left ventricle. This kind of circulation is called double circulation. The two circuits of circulation are the systemic circulation and the pulmonary circulation. The coronary circulation, which supplies blood vessels to the muscular wall of the heart may be considered as a separate circuit. [00:02:00] Systemic circulation refers to pumping of oxygenated blood throughout the body from the left ventricle, and bringing back the oxygenated blood from the body to the right atr. The oxygenated blood from the left ventricle is pumped into the great aorta, which arches itself to the left side and runs down as the descending aorta. The descending aorta through a system of arteries, supplies blood to various parts of the body where they end up in capillaris. The exchange of materials, mainly nutrients and oxygen, takes place between the wall of the capillaries and the tissue. Waste products produced by the tissue cells such as carbon dioxide and urea are collected by capillaries that unite to form veles. The veles unite into larger vessels called veins. Today I'll be talking about biomechanics and functional anatomy of the hip. The hip is the largest ball and socket joint in the body. It differs in design from the more common hinge joints in order to meet the requirements of ambulation. In addition to its stability, the hip joint is also a very mobile joint, second only to the shoulder joint. In the body. The hip joint has three degrees of freedom. That is, it moves in three different planes, sagittal plane flexion, extension, frontal plane, abduction, abduction, and transverse plane, external internal rotation. In the sagittal plane, the hip flexes to 120 degrees and extends to 20 degrees. In the frontal plane, the hip abducts 45 to 50 degrees and AUCs 20 to 30 degrees in the transverse plane.
