Peripheral Neuropathy Relief with Nerve Surgery
Dr. Tollestrup explains causes, symptoms and a surgical solution for patients with peripheral neuropathy.
Eight out of ten patients with peripheral neuropathy who show signs of nerve compression will get relief with pain-focused peripheral nerve surgery.
Peripheral Neuropathy Relief with Nerve Surgery
Tim Tollestrup/Pain-Focused Peripheral Nerve Surgeon:
“Peripheral neuropathy is a condition characterized by painful numbness, tingling, and burning of the feet and or hands.
More people get neuropathy symptoms in their feet than in their
hands but it can affect both and you can have all four limbs affected.
In the context of peripheral nerves, you're sort of dealing with a two-part problem, like a layer cake with two layers.
Very often with people who have peripheral neuropathy there's a metabolic insult going on. That could be something like diabetes, hypothyroidism and maybe exposure to certain chemicals and chemotherapy.
These nerves become swollen and entrapped in tight spaces where they used to fit.
In patients who have peripheral neuropathy what I can do as a pain focused peripheral nerve surgeon is examine them to see if they show signs that there is entrapment of these nerves at these tight spaces.
If the patient shows evidence of compression or entrapment of nerves at certain locations that match up with where their clinical symptoms are, then they're good candidates for nerve decompression surgery.
It is not 100%, some people have the surgery and do not get better but there's nothing else that offers an 80% cure rate besides
surgery to decompress the nerves.
If somebody has an exam and doesn't show signs of mechanical nerve compression, then they're obviously not a candidate for nerve decompression surgery.”

Different Categories of Pain-Focused Peripheral Nerve Surgeries
Dr. Tollestrup explains the wide range of pain-focused peripheral nerve surgeries he performs to help patients from all over the world overcome chronic pain conditions previously considered untreatable.
“Some patients have had serious trauma and have complex peripheral nerve problems involving multiple nerves at multiple levels.
Sometimes you know you end up doing a collection of procedures or coming up with some kind of approach that you may not ever do again or do very rarely.
Most people have problems that fall into basic categories and it's
just a question of figuring out what their problem is and then applying an operation that you've done many times.”
Here are five of the more common pain-focused peripheral nerve surgeries Dr. Tollestrup performs on a regular basis:
1. Piriformis Muscle Removal Surgery
2. Thoracic Outlet Syndrome
3. Superior Cluneal Nerve Removal Surgery
4. Meralgia Paresthetica – Lateral Femoral Cutaneous Nerve Removal
5. Peripheral Neuropathy

Dr. Tollestrup Discusses Chronic Pain
In this episode of Dr. Tollestrup Discusses Chronic Pain, Dr. Tollestrup addresses the difference between pain-focused peripheral nerve surgery and pain management, how each patient's journey is unique and the joy he feels after helping a patient overcome chronic pain.

The difference between what I do, and a pain management setting is that my goal is always to get rid of the pain.
Very often in pain management the goal is to manage the pain; to make the pain go to a a level where the patient can function.
My goal with a patient is to try to get rid of the pain and that's really one of the most rewarding things about this field of surgery.
I love watching these patients’ progress. Some patients are simple and have a simple problem and other patients are very complex.
Some patients fall into categories where there's typical operations that you may do many times because there's many patients with the same problem.
Some patients have a unique problem that may even require a sort of surgery to be developed to treat them.
When these patients come in to see you, you can sort of see this burden of pain that they carry, and I refer to it as the pain mask.
Once you're able to help these patients at the end of the process they look like a different person because that pain burden has been lifted.
In terms of personal satisfaction that's absolutely the thing that is the most rewarding for me.
Most patients who come to see me are as bad as they can be and almost always the only thing I can do is help.
I can't help everybody and not everybody has a peripheral nerve problem. The type of surgery I do is not a silver bullet for every type of chronic pain.
But for patients who have a peripheral nerve problem it's really the only solution that that's out there that can really give them excellent relief of their pain.
