Watch the Vaccine Education video found on the Region of Waterloo Public Health website. Complete the form and electronically sign the attestation after the video to get your vaccine education certificate emailed to you. There is a fast forward option on the video. 2. Print and complete https://mamabearsproject.com/wp-content/uploads/2023/12/on-form.pdf form. You will need one form for each child that needs an exemption. 3. Take the completed form to a Commissioner of Oaths to have it signed and stamped. Photo ID with a signature is required (e.g. driver’s license, picture health card). Some offices require an appointment and some may charge a fee. A Commissioner of Oaths can be one of the following: o Municipal Clerks or Deputy Clerks for your local municipality o Members of the Provincial Government (MPPs) o Provincial Judges o Justices of the Peace o Barristers or Solicitors licensed to practice law in Ontario o Licensed Paralegals o A notary Public 4. Submit your Vaccine Education certificate and the original copy of the completed and signed Statement of Conscience or Religious Belief form to Region of Waterloo Public Health.
