Well, if he’s got private health insurance, his private health insurance may cover it or if your husband is less than 65 years old, the NDIS (National Disability Insurance Scheme) will cover it right for nursing care, 24 hours a day. It is very encouraging to read and listen to the many subjects that you’re posting about when it comes to Intensive Care at Home. Currently, my 42-year old brother has been in intensive care in Melbourne in one of the major hospitals for the last four months. He has a tracheostomy. They organized the Passy-Muir speaking valve, which has been approved by the ENT doctors. The ENT doctors had good things to say about it, however, the speech therapist in ICU only attempted to work with him twice. He’s also now got the PEG tube because they don’t think that he will be off the ventilator anytime soon, if ever. My brother now can follow commands that I have taught him. It is my belief that if he had the therapy needed, he would come off the tracheostomy, off the ventilator and eating as well as talking. But given that, I’m not happy with what he’s getting there in ICU. I want to bring him home as quickly as possible. So what do I need to do to bring him home and who’s going to pay for it? I do hear that the NDIS (National Disability Insurance Scheme) is paying for some of your client services. Can you help us with this situation? From Carra Hi Carra, Thank you so much for writing in. So Carra, I can’t comment on what’s happening there in ICU but what I can comment on is that obviously the longer someone stays in ICU with ventilation and tracheostomy, people are getting depressed. ICUs have probably more urgent things to address, meaning, a long term patient like your brother is, may not get the attention he actually needs. He probably gets way more attention at home with a designated team, 24/7, 365 days of the year. I think that might be your next step. Yes, the NDIS (National Disability Insurance Scheme) will be paying for it. Given that your brother is 42, he will be qualifying for NDIS. And yes, you’re right, we are advertising for ICU nurses in Melbourne and in Sydney, because more and more clients want to take up our service because they want to leave intensive care, which makes a lot of sense. So the next step really is to talk to us. We can help you take the next step with the NDIS. We can help you with Specialist Support Coordination, which leads often to funding. And then, the next step is to talk to the ICU, to obviously exercise your and your brother wishes. Then we can look at, equipment at home. We can look at ventilator, suction machines, monitors, a special care bed like a hospital bed, hoist, or lifting machines at home and building a team. Now, this all takes a little bit of time, but it’s obviously absolutely doable. And also the ICU should be supporting your brother going home because it frees up their ICU bed. And it frees up some of their staff and it saves them a lot of money. As a matter of fact, it saves them about half of the cost of an intensive care bed. So it’s a win-win situation for everyone. So, in today’s video, I want to actually answer a question from one of our readers and also potential clients who asked us whether we provide home care nursing services for TAC (Transport Accident Commission) clients in Victoria. A short answer to that is this, it’s a YES. Background of the question is that a lady has contacted us who has a husband in ICU after a motor vehicle accident and he’s now on a ventilator in intensive care. And he’s struggling to come off the ventilator and she’s asking whether he should have a tracheostomy. And if he has a tracheostomy, if he then can go home with Intensive Care at Home funded by the TAC. Now, we are not in control yet in Intensive Care at Home on what the TAC is and isn’t funding. However, the short answer to your question is absolutely yes. We do take patients on from intensive care with a ventilator or a tracheostomy, whether it’s invasive ventilation with a tracheostomy or whether it’s noninvasive ventilation such as BiPAP/ CPAP ventilation with a mask. Now, in this situation, we have looked at the situation of the lady and the gentleman probably doesn’t need a tracheostomy, which is great. Hopefully he’ll come off the ventilator and then go on to rehabilitation and go home. However, we are currently looking after TAC clients who are ventilated with a tracheostomy at home. We have also looked after TAC clients over the years who didn’t have a tracheostomy but needed BiPAP/ CPAP ventilation after motor vehicle accidents. We also looked after TAC client that weren’t ventilated but had a tracheostomy 24 hours a day. So we’ve really covered a whole range for TAC clients again, for those of you who may wonder what does TAC stand for. TAC stands for the Transport and Accident Commission in Victoria. So I hope that answers your question. If God forbid you are in a motor vehicle accident in Victoria and you need TAC support and you need nursing care at home from the TAC and you have that prescribed, you should contact us for nursing care at home with Intensive Care at Home. Or if your loved one is stuck in ICU after an MVA (motor vehicle accident) long term because they can’t come off the ventilator or they need a tracheostomy, please contact us. Now, in today’s video blog, I want to answer a quick question from one of our readers, John. John is 85 years of age. He says, “I have been diagnosed with B-cell lymphoma, and since then, I’ve had two tumors on my lung. Now I have a nodule on my right lung and a nodule on my tracheostomy. And I have been having a tracheostomy for the last two years. I’m 85. Should I have surgery or should I just go home and have palliative care?” Now, John, I am very sorry to hear about your situation and it’s a very difficult question to answer here and, I am not an oncologist. I am not a cancer specialist here or you probably need some involvement from a cardiothoracic surgeon. You probably need some input from ENT, from oncology, to see what your next steps are. Now, from a care perspective, if you do have surgery and then you need to go home potentially with a tracheostomy or on a ventilator, Intensive Care at Home is certainly the right place to do that. We also look after some clients at home that are not ventilated but still have spent extended and prolonged periods in ICU/PICU for significant medical complexities that warrants for them to have a team of ICU/PICU nurses 24/7 at home. They often have neurological conditions with seizures that require unstable airway management at times. Unstable airway management is a skill that only critical care (ICU/PICU/ED) trained doctors or nurses have. In order to improve our clients and their families quality of life/quality of end of life quickly, the only way is to get them out of intensive care back home with the same level of skills and expertise than they get in ICU/PICU. In essence, intensive care at home provides an evidence based intensive care substitution service with the primary goal to improve our clients and their families quality of life. We are looking after patients at home that have a tracheostomy and we’re looking after them at home 24 hours a day because with critical care nurses or with intensive care nurses with a minimum of two years ICU or critical care nursing experience, because that is what is needed and is also evidence-based. So, you already know that your dad is at high risk of potentially dying with a tracheostomy, especially if he’s a quadriplegic and he can’t maintain his own airway. And he can’t manage his own airway in case of a medical emergency, which can happen at any given time with a tracheostomy. In today’s blog, I want to focus more on motor neuron disease patients and truth if they have a tracheostomy or not. So after having worked in intensive care and also with intensive care at home for over 20 years, I have found very different approaches when it comes to tracheostomies for MND or motor neuron disease clients. When I first started working with intensive care at home in Germany over 20 years ago, we were looking after many MND clients at home that had a tracheostomy and they overall reported a very good quality of life or quality of end of life at home rather than spending time in ICU or being denied a tracheostomy in the first place and basically often dying a long and painful death on non invasive ventilation such as BIPAP or CPAP. The best example to give you there is probably Stephen Hawking. Stephen Hawking lived with MND for decades on a ventilator with a tracheostomy and look at his contribution to science and to humanity. And he really lived life to the fullest, even though he had many perceived limitations with MND, he was paralysed, ventilated and had a tracheostomy. So with intensive care at home with what we’re currently doing, we’re also having some clients with MND on ventilation with tracheostomies. But unfortunately, my experience here in Australia is that, not many patients with MND actually get offered a tracheostomy. And I believe, there is clearly some misinformation for patients that suffer from MND because very few MND patients get offered a tracheostomy. This is wrong on all levels and MND patients need to be given a choice. As the disease is progressing and the ability to breathe is fading away MND patients need to be offered a tracheostomy and ventilation so they can actually have a choice to prolong their life and go home with a service like intensive care at home. The ICU says he will be in ICU for many months. Can he go home instead with Intensive Care at Home? The short answer to this is an absolute yes. I have looked after so many patients in intensive care with Guillain-Barre Syndrome that are there for many months on end to be weaned off the ventilator often successfully, but it takes a lot of time. Patients are in ICU in an environment that’s not conducive to recovery. It’s not a holistic environment. It’s an environment that is noisy, loud. There’s no natural daylight. The lights are on all day long, 24 hours a day. There’s people running around 24 hours a day. It’s not a conducive environment for someone who needs long-term intensive care, which is what’s happening with all Guillain-Barre patients when they are in ICU on a ventilator with a tracheostomy, they need long-term intensive care to get them off the ventilator. And that’s when Intensive Care at Home is a really good solution.
