We were asked to provide our insurance information. We also contacted our insurance prior to arriving just to make sure they cover and the number of times we can take the test.

The process:
1. We submitted a Drive-Up Registration Form (not required for established patients)
2. We submitted a COVID-19 PUI Form (required for ALL)
3. We picked available Date and Time (I'm not sure if it is required)
4. We were assigned a Parking Space from 1 to 4 (required for ALL)
