I don't get the (Secret) Survivor Achievement for Completing the Game Without Dying, but provide a workaround that you can try, to help get it. I'm don't know if this workaround will work on the PlayStation version or not. It didn't work for me on the PC version, but I might have also messed up in doing it.

This Walkthrough wasn't made specifically for getting the Trophies on PlayStation, but can still be used to get them.

My Achievement Popups aren't capturing in my recordings for some reason, you can still hear the sound on it popping but can't see them.
I'm sorry for that!!
I'm working on trying to figure out how to get them to capture in my recordings.
