A Family of Lipomas in ONE forearm by Dr. Pimple Popper A lipoma is slow-growing, benign growth of fat cells. It is contained in a thin, fibrous capsule and found right under the skin. Link to Skittle Steatocystoma Playlist: Steatocystoma Multiplex appears as multiple, uniform, yellow, cystic papules usually 2-6 mm diameter, located especially on upper anterior trunk, upper arms, axillae, and thighs. Typically no central punctum or umbilication is seen. Majority of cases present with dermal lesions, but multiple subcutaneous masses looking like multiple lipomas can be present. Bumps usually appear in adolescence or early adulthood, probably because sebaceous activity is at its peak. Sometimes larger steatocystomas are prone to rupture and suppuration and can cause scarring and pain. Steatocytomas typically contain a syrup-like, yellowish , odorless, oily material. If they are inflamed or infected by bacteria, they can develop a foul odor and can be a definite source of social isolation. This is probably the closest thing to a “sebaceous cyst”, a name that many people including non dermatologist physicians may use to describe an epidermoid cyst which we see more commonly. Steatocystoma may at times have vellus hairs, and eruptive vellus hair cysts may have sebaceous glands (oil glands) in their lining, which leads some dermatologists to believe these two conditions may lie along a continuum, that they may be related. Treatment is removal with small incisions and extraction, with care on treatment on the trunk because of the increased chance of poor cosmetic result/scarring. 20 Year Old Fidget Spinner In the Head by Dr. Pimple Popper A Pilar Cyst, aka a trichilemmal cyst, an isthmus-catagen cyst, or a “wen”, looks identical on examination to the more common epidermoid cyst, except that 90% of them occur on the scalp. They tend to be more mobile and firmer than epidermoid cysts, and are derived from a different part of the hair follicle/skin. The internal content tend to be more “homogenous”/ smooth (looks different under the microscope), but it is also a benign growth. It is not uncommon to have multiple pilar cysts on the scalp and to have other family members to have these cysts (tend to run in families). Treatment is for cosmetic reasons usually (people don’t like the appearance of them), and excision with removal of the cyst and it’s contents is curative.

Another name used is “fsebacous cyst” but this is actually an antiquated misnomer, and is not a term used by dermatologists. They are also the most common type of cutaneous cysts. Epidermoid cysts result from the reproduction of epidermal cells within a confined space of the dermis. The pasty contents are mostly composed of macerated keratin (wet skin cells), which creates this “cheesy” consistency, and there can be a pungent odor. An epidermoid cyst may have no symptoms and are typically harmless. Usually people seek removal because they don’t like the appearance of these bumps, or the cyst has ruptured or been inflamed or “infected” in the past. Rupture is associated with sudden redness, pain, swelling, and local heat, and can lead to abscess formation. Also, a history of inflammation, often increases scar tissue in the area, makes the cyst more firmly adherent to surrounding skin, and makes it more difficult to remove. Surgical excision is curative, but the complete cyst removal including the entire cyst sac and contents need to be removed to ensure that the cyst won’t reoccur. This patients came in to see me because she has this "lump" on her upper back that really bothers her. This is a lipoma, which is a benign collection of clonal fat cells, aka adipose cells. They are pretty common, not life threatening, but create bumps under the skin that can grow quite large. They do not cause any pain or discomfort usually, but people seek out removal of them because they are bothered by the appearance of these bumps under the skin An excision is done here, and many times, a lipoma can be popped out whole, because it is often encased in a very thin fibrous capsule. Happy Tuesday to you all! Yes, my kids had plantar warts, which is a very common condition (I had a couple warts on my hand when I was their age). My father is a dermatologist, and I remember him treating MY warts when I was their age with liquid nitrogen, and I guess I'm keeping up with tradition here! Plantar warts, verruca plana, are caused by a virus and a particular subtype of verruca virus likes to cause warts on the feet. Like all warts, they are difficult to eradicate because they are caused by a virus, and this virus is essentially "under the radar" of our immune system. If you are a healthy person with a normal immune system, eventually your body "figures it out" and gets rid of the wart/virus, but the problem is that you don't know when this will happen. This, by the way, is why younger people tend to get more warts in general than older people - because when we are older our body has already been exposed to the wart virus, recognizes it and doesn't let it grow. Which is why I can touch the warts with my hands (even though they are "contagious"), but I don't get warts myself (knock on wood!!:)). My kids are scared (as most children are in general, of seeing the doctor), and I don't know if it's better or worse that their doctor is also their mother! So, I probably speak to them a little different than I do my regular patients. I'm more stern and I tell them to "suck it up", which I wouldn't do to a regular patient (hehe). This patient came in for me to treat her mild psoriasis, which is why you see some redness and skin flaking on her eyebrows and scalp. She also has these white bumps which she tried to squeeze but couldn't remove. PIlar cysts are benign growths most commonly seen on the scalp. They are a little different than epidermoid cysts which are more often found on the body. Pilar cysts usually have thicker walls, so they are more easily popped out whole. As you can see, Dr Rebish and I have different methods for removing cysts, and this is expected since we trained at different dermatology residency programs. He prefers staples in the scalp because these are less likely to snag hairs which can increase chances for local infection. Pilar cysts tend to run in the family.. often more than one family member has them, and it's not uncommon to have multiple cysts. Thank you so much for watching!! This lovely older woman came in to see me for a body exam. She was accompanied by her at-home nurse who visits her at her home daily, to make sure she is ok. Her nurse has wanted to squeeze this bump for many years, but I got a chance to do it, and the patient was so kind to allow me to film it for all of you to see. As you can see I use a punch biopsy tool, 6mm diameter to "punch" out the punctum of the cyst (where I can see the cyst opens to the skin surface). This is really the largest size punch I will use.. If I think I need to make a bigger opening, I will usually opt for an excision with a scalpel instead and this is mainly because if you use a punch larger than 6mm, the area is more difficult to close without getting dog-ears(pooches of skin on the edges of the suture line which distort the skin). As you can see, I was able to squeeze out and remove the entire cyst and its sac and sutured the area closed with deep and superficial sutures because there will be tension on the wound edges and this will more ensure a fine linear scar. This is a young man who had this strange, white, firm bumps, like white marbles under the skin surface. So I excised one of them and sent the tissue for pathology. Results came back as Calcinosis Cutis This results from deposits of calcium and phosphorus in the skin. There are four forms of calcinosis cutis. 1. Dystrophic calcinosis - for example, dermatomyositis, when calcification occurs in damaged tissue, and calcium and phosophorus levels in the blood are normal 2. Metastatic calcification - there are high levels of calcium and phosphorus in the blood and this leads to deposits in the skin. For example, people with hyperparathyroidism can have this. 3. Ioatrogenic/traumatic calcinosis - can be due to medical conditions or trauma 4. Idiopathic calcinosis - which means other forms with unknown causes with normal serum calcium. Essentially, this is applies to all other cases. This patient had normal calcium levels in his blood, and he fits in category number 4. We don't know yet why he is getting these bumps, they are localized to one area of his body, and hopefully we have removed them all (he had two as you see here) and we will continue to watch to see whether he gets more. A patient of mine who was so kind to let me extract the milia in her ears and the blackheads on her back! Thank you for watching! A milium (single for milia) is a small superficial cyst formed in the top layer of the skin called the epidermis. They appear as tiny, pearly-white bumps just under the surface of the skin. They contain keratin (skin protein) and are harmless. They are often seen on the face, in particular on the delicate skin around the eyes. They can be prevented or minimized with exfoliating like chemical peels, microdermabrasion or topical retinoids. However, they are sometimes difficult to remove without a physician’s help, since they cannot simply be squeezed out. Often, the surface of the skin has to be pierced first with a lancet, a sharp pointed blade, or a needle, in order to successfully remove the milium. Often a comedone extractor is used to express the milium. An open comedo (single for comedone) is a clogged pore in the skin that is open to the air. Keratin (skin protein) and sebum (oil) combine to block the pore. They are often found on the face and trunk. but they can be found anywhere on the body. Blackheads are not clogged with dirt, but it is the exposure to air that causes oxidation turning the internal contents black. They can be extracted using a comedone extractor. I usually use an 11 blade (a blade that comes to a sharp point) and a Shaumberg type comedone extractor.
