REAL ESTATE NEWS (Los Angeles, CA) — While real estate in the U.S. and other western countries crashes up today due to inflation, China’s real estate market was so extremely overbuilt that its prices are now tumbling dramatically. The communist regime does not release honest statistics, thus the books of western companies doing business in China provide the only accurate picture of China’s true economy. Recent new lockdowns also reveal the true story. The regime of nutcase dictatorship is seriously worried about recent bad economic results, thus uses today’s most convenient of scapegoats: covid hysteria. The real estate market is still hot in many U.S. areas as families and investors all scramble to buy physical assets that can greatly hedge against runaway inflation, and protect against a falling dollar. In many other U.S. markets, a quiet crash or softening is already taking place, unreported in the news, and obscured by inflation. Indeed, a stagnating overall economy, Greater Depression of the 2020s, has been hidden behind a curtain of inflation for several years. One thing that can pierce the veil is a truly committed, rapid-acting Fed, which we don’t have while interest rates are still near historic lows, and government spending and money printing are still near historic highs. Concealed stagnation, government overspending and ballooning hyperinflation are still the name of the game for real estate and most other markets in the United States. Governments worldwide are failing to address the real issues, as they rely on fear and distraction, amid calls for optimism.
What is going to be the cure or the weapon that beats hyperinflation? The US Dollar is a goner for many reasons: The Fed is not keeping pace, and will not keep pace required to beat hyperinflation; the overused, abused US Dollar is losing its global dominance as the monetary standard; the dollar is being replaced by blockchain cryptocurrencies — and good money destroys bad. In addition to optimism, individuals, families and companies must own physical assets that perform well during inflation and stagnation, such as real estate, gold, farm land, blockchain cryptocurrencies, quality stocks, technology patents etc.
A worldwide economic depression began in April of 2020. China and the U.S. lead the way in the latest “cures”: denial, deception and cover-up. The key difference between U.S. thin veil of real estate health, and China’s undeniably poignant real estate crash: the unprecedented level of ponzi scheme activities in China real estate vs the more moderated, varied markets of America. The U.S. does have its own looming ponzi crash coming: the end of the global hegemony of the U.S. Dollar, amid emerging general demise of fiat currencies. Unlike communist China’s housing implosion, real estate in the U.S. today still provides an effective shield against these economic threats.

The biggest risk for the stock market in 2021 is inflation, according to Morgan Stanley. Unprecedented radical spending by the federal government and the Federal Reserve, to stave off a panic-induced market crash, helped artificially drive stocks to temporary new highs last year. Morgan Stanley analysts worry that the unintended consequences of money printing and Fed spending sprees could abruptly change markets for the worse.
The biggest surprise of 2021 could be higher inflation than many, including the Fed, expect. The Fed’s massive spending during the virus hysteria has moved beyond simply filling holes left by exaggerated crises. While some mistakenly believe that we’ve seen a recovery, the tens of millions of unemployed and underemployed Americans feel otherwise. While some industries and monopolistic companies are doing better than ever, most medium sized and small business suffer, or go out of business completely.
By buying back $1 trillion in securities, the Fed has created a market that’s awash with cash, which historically helps drive inflation. Morgan Stanley warns that drunken-sailor Fed spending and helicopter handouts may drive up prices once the pandemic subsides. For food, household goods and building materials, inflation already hit well above 10% in 2020. Home builders report wood 2x4s and other construction commodities recently tripled in price. Increased construction costs contribute to increasing or flat home prices while the real value of the home may be decreasing, especially for neighborhoods with increasing blight, crime and homelessness, amid decreasing employment opportunities.
Many do not realize that prolonged artificially low interest rates create tremendous building of opposing pressure toward inflation, which can happen abruptly. This could mean very bad news for those who are overextended on short-term credit, such as home owners with variable rates and investors with balloon payments. Commercial real estate investors are already very hard hit.
The pandemic crisis, which thousands of censored medical doctors and health experts have labelled a hoax, was “perversely” positive for large companies. Epidemiologist scientists, including Professor Mark Woolhouse have warned that panic-induced overreactions including lockdowns have been a monumental disaster on a global scale, which should never been repeated.
The Federal Reserve is buying back $120 billion of Treasury bills and mortgage-backed securities each month, after initiating a massive $700 billion asset purchase program in March. The U.S. federal government, meanwhile, has authorized some $3.5 trillion in spending to shore up the economic recovery as a result of the pandemic. Trillions more of additional federal hand-outs are in the works.
President-elect Joe Biden nominated former Fed Chair Janet Yellen to head up the Treasury Department, a sign that the federal government could more towards universal basic income, the most colossal ejaculation of cash in history. That would create direct, explosive price inflation, exactly the sea of change that can lead to unexpected outcomes in the financial markets.
As the dollar rapidly loses value to blockchain cryptocurrencies, Morgan Stanley has recently increased its holdings of Bitcoin bulls such as MicroStrategy.

From Transplanted to Transformed
There are three-hanky stories, and then there are those that force you to make a run to Sam’s Club to buy Kleenex in bulk, like the one Oscar tells about his daughter Kairi in the moments before she was taken away for transplant surgery. By Jeff Weinstock
“I don’t know if I can get through this because it’s kind of hard,” he says preemptively, and then begins. “Kairi said she had some letters for us. She said, ‘You can’t read it! You can’t read it until I’m in the operating room.’ She gave one letter to me and one letter to my wife. She said, ‘Promise me you won’t read it until I’m in the operating room.’
“We went to get some coffee because we knew it was going to be a long night. We sat down and started reading the notes. On my note it said, ‘Daddy, don’t worry. Don’t get anxious. Everything’s going to be OK. I’ll see you after the operation.’ I still have it. I carry it with me all the time. She knew I get anxious when it comes to her. That put me at ease.”
When she handed him the note, it was folded and sealed with hospital tape, the kind used for binding up wounds, or securing a father for a long night ahead.
Fourth time’s the charm
By his own admission, Oscar frets easily. He wasn’t built for the strenuous wait that transplant candidates go through, drawn out to 18 months by the need to find a double-organ match to replace Kairi’s diseased kidney and liver. He’s the one who fielded the three previous phone calls from the Children’s Hospital Los Angeles transplant coordinator, with word that a donor had been found.
On each occasion, as the family was warned could happen, the offer fell apart, once because Kairi was second in line and the patient ahead of her accepted it; another time because the organs after investigation were deemed flawed; and the last time because the donor family had a change of heart.
But Oscar sensed this call was the one. “For some reason, I don’t know what it was,” he says, “it was a little bit different.”
Confirmation came when the transplant surgeon himself, Kambiz Etesami, MD, walked into the end of Kairi’s dialysis session with the consent forms for the family to sign. The operation was set for the following day.
Walking to the OR, “I couldn’t even talk,” Kairi’s mother, Roxana, says. “I was so overwhelmed. I was thinking, ‘Oh my god, this is it.’”
The surgery began on the evening of July 17 and ended the morning of July 18, spanning 14 hours in all. Outside of bathroom breaks, the only time Dr. Etesami stepped back from the operating table was to check that blood flow to the new organs wasn’t “kinking,” he says, and to see if the implanted kidney and liver were functioning. The donor was a small adult, so he had some concern about the fit in Kairi’s 11-year-old body.
“We thought she had enough room,” he says, “but you never know until you’re there. Her abdomen actually closed smoothly at the end.”
Finally, an answer
The follow-up on the procedure has been intensive and shared. Dr. Etesami stayed on for the first two precarious weeks, but his part has mostly ended, as Kairi’s kidney and liver specialists, Rachel Lestz, MD, and George Yanni, MD, respectively, now see Kairi in clinic frequently to monitor her organ function and her lab work.
The most significant result to come since the surgery was the apparent pinpointing of the source of Kairi’s liver and kidney failure, which had eluded the two doctors’ painstaking efforts to uncover it, having not surfaced in the earlier biopsies or genetic testing. The pathology on the explanted liver—the one that was removed—indicated autosomal recessive polycystic kidney disease, or ARPKD, a congenital kidney disorder that also affects the liver and, Dr. Yanni says, necessitates a double transplant in 20-40% of patients.
Adding things up retrospectively, both doctors had belated aha moments.
“I went back and looked at my original consult, and ARPKD was in my differential as a possible solution as to what could bring all these things together,” Dr. Lestz says. “I thought, ‘Oh, OK. That makes sense now.’”
If the finding can be corroborated by CHLA geneticists, as Dr. Lestz hopes, it’s an ideal outcome, as ARPKD can do no further harm to Kairi. It was erased for good once her native liver was taken out. There’s no threat of a post-transplant recurrence.
In fact, there’s a decent chance Kairi can stay out of harm’s way entirely if she keeps to her medication. She will have to take immunosuppressant drugs indefinitely to restrain her immune system from feasting on the new liver and kidneys it sees as trespassers. Dr. Etesami says some patients are fortunate to achieve tolerance, where their bodies come to accept the new organs as their own. But the majority can only hope for a workable truce. After a year or two of cohabitating, “usually the body and organs start to get along,” Dr. Yanni says.
Certainly Kairi has welcomed them like they were her adopted kin. She named them after her doctors. Naming your transplanted organs “is sort of a thing,” Dr. Lestz says.
The new organs both saved Kairi and revived her. It’s more than metaphor to say she seems to inhabit a new skin. Always dry and itchy, her skin is now a robust pink. She bounces from one thing to another, writing stories, playing with her dogs. Before, she stayed mostly in bed, tired and glum. She’s still too vulnerable to infection to get back to her sixth-grade class, so she’s being taught at home, with the intention for her to return to school in January.
She has returned to two of her favorite things that had been denied her. One was chocolate, whose high phosphorus content her diseased kidney could not filter out and made it a strict no. The second was swimming, which was forbidden because she couldn’t get her dialysis port wet. The family pool had been shuttered altogether since January 2017. If one couldn’t swim, then they all weren’t going to.
Soon after returning home from the transplant surgery, she went in the pool with her sister, though went in may be a little gracious. “She pushed me in,” Kairi says.

As a journalist to this day, I have a responsibility to report all truth that affects DTLA real estate. I’m still known by some as an experienced medical device marketing professional. I should write about covid ventilator deaths because I am among the few who can. Active licensed medical practitioners are today being fired for telling the truth. In general, they are not allowed to provide you with any information that may differ from that which big pharma orders them to say. I have the data, experience and capacity, and I have no risk of losing a medical license, because medical marketing specialists need no license. Other reasons why I write this: Mass hysteria has directly affected real estate, primarily in urban neighborhoods like Downtown Los Angeles, along with other major metropolitan areas now experiencing economic losses and increasing crime related to panic, fear and related re-suburbanization. Most importantly, because I happen to have sets of very relevant experience, personal and professional. In a marketing career spanning nearly 50 years, I’ve helped to market, advertise and sell medical equipment, including Newport NMI ventilators. I’m very concerned when I hear numbers of respiratory therapists dramatically express their concerns that they are being ordered to kill covid patients by using improper procedures and protocols.
Working directly with RT Respiratory Therapists and ventilator manufacturers, I’ve learned that ventilators are nothing to sneeze at. They are one of the most critical, life-or-death medical technologies on the planet. They breathe for people who cannot breathe for themselves. If the patient is suffering respiratory distress, can breathe for themselves, but not registering a specific questionable level of oxygen, doctors will often paralyze the patient’s airway so that the patient can no longer swallow, cough or breathe. If the intubated patient does not receive sufficient attention, if the therapy is improper, or if the machine is set to the wrong setting, the patient dies. If the therapy is not quite perfect, the patient dies. If the patient has a very bad cold or pneumonia, where the patient has liquid or phlegm in the lungs that must be expectorated (normal for a bad cold or flu), the patient on a ventilator cannot expectorate, cannot achieve proper oxygen levels, thus suffers a stunningly high chance of death. Therapists have been ordered, and some are still being ordered, to crank up the ventilation pressure higher and higher, even though the therapist is fully aware that the patient shall deteriorate toward demise with this therapy.
Medical professionals bear no risk in providing these horrific procedures to Covid patients. The greedy, hasty medical culture of today is set up overwhelmingly in favor of treatment and over-treatment, with little regard to patient outcomes. Medical professionals are today only at risk if they fail to treat. They are protected if they over-treat, and they are harshly punished for disagreement or disobedience. In an era of intense censorship and massive pro-pharma propaganda, the careers and livelihoods of medical professionals are on the line if they tell the truth. Fortunately, more than one brave (and/or foolish) nurse, RT Respiratory Therapist and doctor has ventured to inform the public. A few have stepped forward to admit to the fear and horror involved with subjecting recent patients to a 100% chance of death through this highly fatal medical treatment. Many of the deaths are preventable simply by avoiding ineffective treatment. Professionals who use a more traditional approach, such as nasal cannula oxygen, report a much better than 50% survival rate. Patients who receive no treatment at all have often seen the best results. RTs have been trained to cajole the patient into ventilator therapy by telling the patient that they will die without the ventilator. The therapist knows, however, that the patient will likely die with the ventilator, up to a 100% chance of death by ventilator in recent instances. Concerned RTs have reported that, for most covid patents and oxygen, less is more. Due to today’s radical culture of rampant over-treatment, shockingly aggressive propaganda campaigns and extreme censorship, proper changes to respiratory care are not being promptly implemented.

I have a unique vantage point from which to see the totality of pandemic hysteria. With a background in medical technologies, and with one of the largest databases in the real estate industry, I have unique access and communication with tens of thousands of individuals. In it, the number of young, totally healthy individuals who have died of today’s bad flu season is very close to zero. Compare this to historic epidemics, such as the Black Death bubonic plague of 1350, which killed nearly 1/3 of Europe. Today’s “plague” standards are artificially low — nothing remotely like an historic blood-borne plague that wiped out more than 30% of some populations in the past. The CDC reports that today’s airborne pathogen pandemic kills far less than 1% of the population — much closer to 0% of the population.
We’ve seen and heard of others who have have had covid, usually undetected, with mild symptoms or no symptoms at all, and several with very bad cold symptoms complicated by panic and over-treatment. A few of whom we have heard have actually passed away — mostly from old-age related diseases, obesity-related diseases or hospital mistakes. These are not reasons to create more mistakes and to destroy more lives.
Put things into perspective. Compare 30%+ death rate of the Black Death to approximately 0% death rate for Covid-19. The most massive blood-borne pandemics in history killed many, but even the worst bugs could not kill 100% of the population. Today’s relatively very weak respiratory pandemic utterly lacks the deadliness of historic blood-borne pathogens. On top of that, respiratory viruses have always, always burned themselves out long before they could possibly kill a large percentage of the population. By any reasonable standards, the death rate is low, and respiratory viruses are NOT a significant threat to humanity. To verify this, look at the total excess death count, which is similar to previous bad flu years, and look at the perplexing disappearance of flu deaths and pneumonia deaths. Where did they go?
Today’s total death count is in no way similar to Black Death years. In fact, Dr. Birx admitted to a “liberal” policy regarding comorbidity. She and the CDC admit that more than 94% of “covid” deaths are linked to other deadly diseases and other deadly pre-existing conditions. No matter how the profiteers try to hype it up, covid is NOT the leading cause of death in the world today. The leading causes of death are today ignored, neglected in favor of the exciting disease du jour, Covid and its sparkling new variants. The real, leading causes of death are not only ignored, but actually encouraged — did you get your free donuts? But that’s not all! Coming soon: Get 10 free booster shots and 10 free donuts! What a deal! Or is it?
Unhealthy government propaganda and health care profession promotion of bad health is nothing new. Did you get plenty of DDT and fresh Camel cigarettes when they were promoted by the government and doctors decades ago? Did your great grandparents take their proper doses of snake oil, heroin and cocaine in the 1880s? How about shock treatment, leach or lobotomy, dissection of your brain to lift your spirits? They were all commonly promoted and prescribed by government, medical doctors and salesmen alike.
There was one questionable treatment device that might actually be even more appropriate and needed today: the hysteria vibrator. At least we know that the vibrator will not cause shoulder and arm injury SIRVA, guillain-barre syndrome GBS, acute disseminated encephalomyelitis ADEM, idiopathic thrombocytopenic purpura ITP, paralysis, permanent disability and death.
Take any reasonable glimpse of the negative effects of over-reaction: the devastation of urban centers with riots, crime and collapsing condo prices, the real deaths of countless unreported lockdown casualties, including my own Uncle Lonnie and Cousin Gavin, killed by lockdown depression; the countless untold stories of child suicide, worldwide starvation and other unreported horrors. Our government and healthcare industries have starkly refused to conduct even the most reasonable survey of the adverse health, economic and social consequences of lockdown and other government overreach. Flipflop Fauci and others flatly refuse to count or consider the dire negative consequences of their actions. They refuse to account for deaths, disabilities, anxiety, accidents, riots, explosions, poverty, pollution and other suffering caused by overreactions that they have purposely frothed up to benefit their own profit and power. They ignore an 80% to 100% ventilator death rate. They ignore the destruction of millions of small businesses, devastation to the middle class and worldwide poverty. The perpetrators have thus far taken no responsibility for nonsensical overreactions and unbridled mass hysteria, which characterizes today’s ongoing pandemic of what good, honest doctors have sanely labeled a “bad flu season” — ethical doctors who are being censored, threatened, harassed and fired like never before in history — for telling the truth.
If the medical industry is willing to continue to provide ventilation therapy with a 80%+ average death rate, imaging how high of a death rate they are willing to accept from experimental (and extremely profitable) vaccinations? We already have some answers to this deadly question. Government and the health care industries have a long history of being astonishingly willing to accept a sky-high rate of death and injury for therapies with little or no efficacy. Whenever the public allows it, government officials and the medical industries have a long history of being wrong, of promoting fraudulent epidemics with fraudulent cures. This is nothing new. Remember the Swine Flu vaccine fraud of 1976.
The absolutely atrocious percentage of ventilator deaths are only one example of countless instances of psychotic behaviors that make up today’s frantic stampede of mass hysteria. More than anything else, they want you to lose faith in your own body and its natural ability to benefit from trillions of viruses. They don’t want you to know that the very foundation of your health is based on beneficial bacteria and viruses. They only want you to know what fits the desired political narrative. Today, any news that doesn’t fit that narrative does not make it to the prime time news.
The human body is comprised of more bacteria cells and virions than human cells. We are bacterial and viral creatures. Medical doctors are only just barely beginning to learn of the very deadly consequence of antibiotics, which kill the body’s vital, natural ecology of beneficial bacteria. Thousands die every day from over-medication, over-treatment and inappropriate treatments — almost all covered up. Doctors routinely fail to warn patients of the true nature of side effects. The truth: take too much antibiotics and you may find yourself bleeding from nearly every orifice because you haver killed your beneficial bacteria, thus invited the bad bacteria to take over. Most medical doctors receive very little education on viruses. The medical establishment and the public are today vastly uneducated, illiterate and profoundly uninformed about the vital nature of the human virome. Still wallowing in the victorian era, the health care industry is grossly uneducated and ignorant about the symbiotic relationship between humans, the common cold and countless other viruses. We are bacterial and viral creatures (along with yeast, fungi, mites and many other vital creepy crawlies). You and I are made up of 380 trillion viruses. Colds and flu are part and parcel to humanity. Without colds and flu, there are no humans — and vice versa. With today’s medical technology, the only way to fully prevent bad flu seasons is to kill all humans. Today, we are allowing runaway quackery — false cures that are far worse than the disease.
Today’s ongoing crusade of mass hysteria sadly marches us all down the wrong path, the path to worsening health. Like the respiratory nurse who reports to lament her recent 100% ventilator death rate (yet keeps right on killing), the medical industry gets paid to provide us misinformation about our own bodies. The current state of public health emergency provides extra payment to hospitals, incentive to over-report and over-hype, plus extra incentive to kill us with ventilators.
The good doctors (who are being censored), try to remind us that viruses are part and parcel to our good health. They are being silenced. They are unable to get out the truth. Our bodies are made to handle colds, flu and pneumonia, even as some unhealthy and elderly often naturally pass away of these illnesses. The bad doctors, who are presently in charge, peddle panic, obfuscate comorbidity and attempt to force experimental treatments and other improper therapies.
While it may be understandably difficult to tell when large numbers of people are behaving in a manner that is counter-productive, you can identify mass hysteria by these collective obsessional behaviors, crazes and abnormal group beliefs: 1. Based on panic or fear, large numbers of people take actions that may be detrimental to their own health, welfare and well-being; 2. Large numbers of people ignore, discourage or even hide information that may be beneficial to their own health and well-being, embracing censorship and propaganda from perceived leaders and charismatic figures; 3. Large numbers of people undertake unnatural actions, such as prolonged wearing of masks that reduce oxygen and increase consumption of human wastes including carbon dioxide etc; 4. Large numbers of people seeking medical treatment for symptoms that are normally treated at home, such as sniffles and cough, risking death from over-treatment and hospital mistakes; 5. Large numbers of people being misinformed that they are sick or at extreme risk of illness until they begin to believe it; following strange fads such as face masks; riots. Sound familiar?
Another reason why I write this article, from first-hand experience, is that I and healthy friends who I know, who have had the very bad cold, and let our bodies deal with it the natural, healthy way — we are all very much alive and stronger than ever. Some of those who had pre-existing conditions, overmedication or inappropriate medical treatments are dead. The tragic deaths of the few are then being exploited politically to increase medical tyranny, while the stories of millions and millions of healthy people, who have had the virus with little or no illness, are being covered up. Countless deaths caused by overmedication, pre-existing conditions, inappropriate medical procedures and hospital mistakes are likewise being covered up.
I know the good medical professionals. I’ve worked with them for years. The good doctors are honest, they’re against hype and hysteria. The good doctors have good results. But, the good doctors are NOT presently in charge. Corrupt, inhuman corporate beasts control your health, along with their partners in crime, drooling power-hungry politicians, are in charge of the world’s health, in charge of Downtown Los Angeles real estate, in charge of your dwindling dollar value. As they forcefully deny us a second opinion, they admittedly snuff out 50% to 100% of the lives of ventilator patients, these corrupt creeps are increasingly in charge of our once-independent lives. They freely admit to caring zero, zilch about the casualties, the atrocities: depression, global poverty, injuries, deaths and disasters linked to their misdeeds. Corrupt politicians and their armies of greedy quacks will never willingly hand us back power and control.
Some say that the details of the pandemic are even more sinister and diabolical than we might imagine. Regardless of our varying beliefs, history shows that destruction of free speech ALWAYS leads to atrocities. If we wish to avoid denial of basic rights, among other lingering catastrophic consequences of medical tyranny, we MUST take back our God-given liberty and health, by all means necessary. — Corey Chambers, Los Angeles

Why I Support Children’s Hospital Los Angeles
I grew up right here in Los Angeles. Born right nearby at St. Francis Hospital. I remember when I first heard about a young person close to our family suffering from a nasty disease and getting treated for that at Children’s Hospital Los Angeles. A Stranger’s Gift Leads to Milestone Liver Transplant
A baby in California needed a liver. A man in Ohio—grateful for his own second chance—gave him a piece of his own.

You could begin the story of the 400th pediatric liver transplant at Children’s Hospital Los Angeles in a lot of different places.
You could start, of course, on Sept. 30, 1998, when the Liver and Intestinal Transplant Program performed its first living donor liver transplant—led by Yuri Genyk, MD, the same surgeon who has since led 400 transplants.
Or, you could start on Dec. 15, 2019, when parents Aileen and Chad noticed that the eyes of their 2-day-old baby, Jacob, had turned yellow.
But really, this extraordinary story begins much, much earlier. It starts on Feb. 17, 1969, on a cold, icy Monday morning in the town of Mansfield, Ohio, with a group of six children walking to school together.
The children—all siblings—are traipsing up a steep hill. A boy, 12, is in the lead. But as he crosses the street, a car suddenly speeds down the hill.
The driver does not see the boy in time.
Mike’s story
The boy who got hit by a car all those years ago—he flew 54 feet before crashing to the concrete—was Mike Speck.
He was in a coma for two weeks and spent three months in the hospital. But that was just the beginning of his long road back. He had to re-learn how to talk, walk, eat, read, write—everything.
What’s more, he’d lost all memories from his first 12 years. He had to be told who his parents were, his siblings, his friends. Before the accident, he’d been a top student and an avid baseball and basketball player. After the accident, all that changed.
“I went from straight-As to Ds and Fs, barely passing, because I couldn’t remember anything,” Mike says. “I couldn’t even walk 50 feet until my senior year in high school. It was a tough time.”
Still, his family—his parents, his brothers and sisters, his grandparents, aunts and uncles—all rallied behind him. And slowly, amazingly, he healed. Not only was he able to eventually walk and run again, but by his 20s, he was playing in a softball league and earning a 3.4 grade point average in college.
From there, his life took off. In the decades since, he’s been a chemical engineer, the owner of a popular pizzeria and a devoted basketball coach at a local school. He has a wife, Genevieve, four daughters and seven grandchildren.
Mike describes himself as a very spiritual person. And through the years, a question gnawed at him.
“Why did God let me live?” he wondered. “Why was my life spared?”
Jacob’s story
On the day that Aileen and Chad noticed their son Jacob’s yellow eyes, they weren’t too worried. Their older son, Lucas, had been jaundiced after birth, and after some simple phototherapy treatment, he had been fine.
The next day, a Monday, they took Jacob to his pediatrician. By that night, Jacob was hospitalized in San Diego, about 100 miles from the family’s home in Imperial County. The next morning, he was transferred by ambulance to yet another San Diego hospital.
“That’s when we knew it was something serious,” says his dad.
That “something serious” turned out to be biliary atresia, a rare condition where the liver’s bile ducts are missing or blocked. When Jacob was 6 weeks old, he underwent surgery to try to repair the problem, but soon he was in and out of the hospital again.
His belly would fill with so much fluid, it swelled up like a balloon. Worse, that fluid often got infected. His parents learned to look for the telltale sign that they needed to take him to the emergency room: He’d stop smiling.
3-of-us.jpg“Even with a distended belly, Jacob would be happy, but when he had an infection, there’d be no smile,” Chad explains.
In June, Jacob was referred to Children’s Hospital Los Angeles for a liver transplant. His parents met with Dr. Genyk, Chief of the Division of Abdominal Transplantation at CHLA, who has led the Liver and Intestinal Transplant Program for 22 years. Dr. Genyk partners with Beth Carter, MD, who is Medical Director of Liver Transplant and Intestinal Rehabilitation.
The program performs 25 to 30 transplants a year—the most in Southern California—with survival rates that exceed national averages. One third of those transplants come from living donors.
“The advantage of the living donor is that you can do the transplant before the patient’s condition really deteriorates,” Dr. Genyk explains. “Outcomes from living donors are generally superior to cadaveric donors, and that’s especially important for pediatric patients. A child needs a liver to last a really long time.”
Luckily, Jacob’s dad was a match. Chad spent a full day undergoing a thorough medical evaluation at Keck Medicine of USC. He thought doctors would just tell him to lose a few pounds. Instead, they had stunning news.
The imaging tests had revealed a tumor growing in Chad’s abdomen. He now had his own medical crisis—and he could not be a donor for Jacob.
“Our son getting sick during the pandemic, and now Chad? I felt like my world was falling apart,” Aileen says. “If it wasn’t for my faith in God, I don’t think I could have handled it.”
A surprise phone call
Meanwhile, Jacob was getting sicker.
In September, he spent 22 days in a San Diego hospital with a potentially life-threatening infection. He recovered, but it was clear he needed a transplant very soon. Still, he was nowhere near the top of the list to receive a deceased donor organ.
The family stepped up the search for a donor, but it was a slow process. Donating a liver is major surgery, requiring days in the hospital and a weekslong recovery. A few friends had expressed interest, but none so far had been found eligible.
Then, at 4 p.m. on Oct. 12, the family got its most astonishing news yet.
Chad was just pulling into the driveway of his mom’s house when his cell phone rang. It was Children’s Hospital Los Angeles. An anonymous person from Ohio had come forward to be a liver donor. Dr. Genyk had determined Jacob was the best match. Could Jacob be in Los Angeles for the transplant next week?
Anonymous donor? Ohio? Next week?
“Words literally could not come out of my mouth,” Chad says. “Someone in Ohio is coming out to L.A. to save my kid’s life? That’s, that’s—I don’t know what to say. Other than it’s proof that there’s still good in humanity, OK?”
A quest to give back
Three hours earlier, at his home in Sandusky, Ohio, Mike Speck had also received a surprise call from Los Angeles. As soon as he got off the phone, he jumped up from his chair and ran to find his wife.
“Genevieve!” he yelled, breathless with excitement. “You’re never going to guess who just called me.”
Five years earlier, in his continued quest to give back after his long-ago accident, Mike had anonymously donated one of his kidneys. The recipient, a minister, wrote him a letter, telling him the kidney had made him “a new man.”
Mike was thrilled. So when he later found out he could also donate a piece of his liver, he researched hospitals and contacted Keck Medicine of USC. In March 2020, he flew out to Los Angeles for a full donor evaluation. Since then, he’d been waiting for a phone call, hoping he could once again give someone else a second chance at life. Now, that call had come.
He and Genevieve began rushing around, packing their bags and booking a flight and hotel. Two days later, they were landing at LAX.
‘In good hands’
At 9:30 a.m. on Oct. 20, Aileen and Chad saw Jacob off to the most important surgery of his 10-month-old life. Jacob was in good spirits, giving his parents high fives with his tiny hands—and smiling his trademark smile.
His parents headed to their nearby hotel for the long wait. “We knew he was in good hands,” Chad says.
Dr. Genyk led both surgical teams—one for Mike at Keck Medicine of USC, and one for Jacob at Children’s Hospital Los Angeles. In all, the two surgeries took 13 hours.
“Jacob’s transplant was extremely difficult,” Dr. Genyk says. “He was really ill. Because of the infections in his abdomen, it took us three hours just to free up the intestine and liver from all the inflammation.”
But that’s where the experience of 399 previous pediatric liver transplants pays off.
“Reaching this milestone really speaks to the depth and breadth of our expertise,” Dr. Carter says. “When you’ve performed that many transplants, you become very adept at knowing what the potential complications are and how to prevent and treat them if they occur. This allows us to deliver the best possible care for each child.”
That care involves surgeons, gastroenterologists, hepatologists, nurses, radiologists, social workers and more. “Every transplant is a huge team effort,” Dr. Genyk stresses. “The success of this program is because of everyone’s enormous contributions.”
Tears across the miles
A month after the transplant—and less than 24 hours before Chad was scheduled for his own surgery—he and Aileen nervously logged onto a Zoom call. Jacob sat on Chad’s lap, bouncing and smiling and clutching a toy dinosaur.
On the screen waiting for them were Mike and Genevieve.
Shortly after Jacob’s successful transplant, Aileen and Chad had written a thank-you letter to Mike. At that point, he was still anonymous to them, but they had asked if he would be willing to meet them. They wanted to thank him in person.
Because of COVID-19 and the more than 2,000 miles between them, the meeting took place on Zoom. But that didn’t make it any less emotional. The tears were flowing freely in California and Ohio.
“I felt very humbled to see the effects of what transpired after my donation,” Mike says. “To see Jacob, to see his big smile—that was something. It was a tremendous experience.”
Aileen and Chad listened in awe to Mike’s journey.
“It was hearing his story and his motivation to donate that made me tear up,” Chad explains. “He was telling us it was his privilege to donate. We should all strive to be like Mike.”
“We are so thankful Mike came into our lives,” Aileen says. “He gave a second life to our son.”
‘The biggest reward’
Today, Jacob is doing great and cruising around the house with a baby walker. He recently celebrated his first birthday. His smile is bigger than ever.Jake-and-the-Cake.jpg
“Children transform so fast after liver transplant,” Dr. Genyk says. “The biggest reward is to see the outcomes in our patients and witness that event where their life is given back to them.”
Jacob’s family has another reason to smile, too: Chad’s November surgery in San Diego was a success, and he is doing well. The tumor was completely removed, and the post-op biopsy showed it was not cancerous.
“It’s such a blessing to be back to our normal life and to have Jacob healthy,” says Aileen. “We are so thankful to Dr. Genyk and all the doctors and nurses at Children’s Hospital Los Angeles for everything they have done to support us.”
Mike is also feeling great and is back to walking his usual 20,000 steps a day—even in the Ohio cold. He has donated all the organs he can as a living person. But he has another plan to keep giving the gift of life.
He’s already signed up to give blood.

The Wall Street Journal recently reported that we’re at the end of a long walk off a short pier of high budget deficits, into an ocean of inflation — skyrocketing prices for food, gas and housing. Historically, this leads to high consumer prices that crush the economy. The cause? We’re suffering from novel economic oppression — government-induced depression and stagnation. Until free enterprise is restored, we shall continue to see a spastic dichotomy of extreme home prices in suburban developments, along with rapidly falling home prices in sinking major metropolitan centers.
Economic depression, which forces prices down, serves as the primary force masking serious inflation. A better word for this masked inflation is stagflation. In 2020, we entered a worse depression than the Great Depression of 1929. In terms of stock market crashes, unemployment spike and drop in GDP gross domestic product, we’ve dwarfed every adverse economic event in U.S. history. The only reason that the stock market is not in total shambles is because stock prices rise temporarily from direct government overspending, direct fed injections, deficit spending and inflation. The stock market is temporarily disconnected from fundamentals. In fact, Bloomberg calls the situation “the biggest financial bubble in history.” The crashy volatility of 2020 reveals the tremendous strains. This includes crushing downward forces that are being exerted on the markets by the government-caused demolition of Main Street and destruction of the middle class, compounded by the most deep, pervasive and persistent mass hysteria in modern history.
Free enterprise is the goose that lays the golden eggs. Our goose is being cooked. The government has diverted more resources away from small and medium sized businesses than ever before in history. In doing so, they have emptied the pockets of more middle class Americans than ever before. The unintended consequences have already begun to play out in a series of cascading crises, atrocities and crimes against humanity.
Panic causes more harm than good. In extended panic mode, people cause far more danger and damage than they avoid. The economic damage of lockdown, depression and price inflation rapidly trickles down to the struggling and poor. The United Nations has repeatedly warned that the most vulnerable and tragic victims of hysteria and lockdown are the hundreds of millions of children worldwide who are pushed closer toward starvation.
The U.S. and western world is in a quagmire and conundrum of choosing between capitulating to the worse economic depression in history or the illusory boom of unlimited money printing — certain runaway price inflation à la the Weimar Republic. History begs to remind us of the repercussions: When money becomes worthless, holocaust easily follows.
The basic components of home builders, the 2×4, more than tripled in price in 2020. According to Barron’s, the home price surge says inflation is real. The FRED M1 Money Stock chart confirms it: Inflation Monster Has Been Released. Hyperinflation is here. Bond holders beware. The keys to surviving inflation, stagnation and their resulting atrocities? The tried-and-true survival basket filled with cash, low personal debt, assets such as real estate, precious metals, stocks, along with the 11-year proven vaccine against the diseased dollar: cryptocurrencies.
For now, the U.S. chooses the wrong route — banana republic style edicts, censorship, propaganda and cover-up. Deceptive tactics cannot immunize us from economic reality. Only truth, light, capitulation and work ethic can overcome the most imposing of obstacles.

Housing Market During Inflation
Housing market on fire in 2020, unsustainable in 2021 and beyond — without runaway inflation.
For 2021, the real estate market is expected to be more volatile, varied and less predictable than normal. The national housing market is now overheated as compared to five years ago. Greater Los Angeles Area single family home prices are expected to plateau, while urban condominium prices shall continue to fall. Los Angeles, New York and other big city urban center property prices have already been falling for several years.
Price inflation normally has an upward price pressure on most things, including residential properties. Inflation is correlated to a reduced value of the U.S. Dollar. The federal government and Federal Reserve have indicated that they plan to increase spending, increase the national debt, increase the money supply, increase quantitative easing and increase radical new measures to stimulate the economy and liquidity. Most of these measures reduce the value of the U.S. Dollar in the future, resulting in inflationary pressures that building up over time, which can result in sudden, unexpected runaway inflation, immediately followed by rising interest rates. | Blog Video
While the Federal Reserve chairman downplays real estate froth, Chairman Powell agrees that rapidly rising home prices are unsustainable. Many housing markets have doubled or tripled in price since 2011.
More and more economists and billionaire investors have been raising concerns about runaway national debt approaching $28 trillion, tidal wave of corporate debt and crushing personal debt. Unusually large, rapidly growing debts can indicate looming financial disaster in the form of a debt bomb explosion, which could potentially wipe out even the safest of retirement funds. High debt increases inflationary pressure for the future.
The primary counterforce keeping prices down is a declining GDP and surge in unemployment — The Greater Depression of 2020s. Stock market volatility tends to have a neutral impact on the dollar value and housing market provided that the long-term end result is near normal.
As 2021 is largely a repeat of 2020 in many ways, we can see most home prices continue to accelerate only if the federal government and Federal Reserve loosen purse strings even more than they did in 2020. These extreme measures are unsustainable, as 2020 already created price shocks in real estate and in many products while the average income went down, resulting in reduced productivity and declining net worth for the average American. Inflation is linked to inefficiencies and detrimental effects such as currency crises, shortages and decreased overall standard of living. The only way to keep pumping up the housing market is by pumping up inflation and costs of living in general.
The most practical ways to protect one’s finances, and to benefit from inflation, is by owning assets such as gold, bitcoin, stocks and real estate. Renters and paycheck-to-paycheck consumers tend to suffer the most when severe inflation takes hold.

Q: What’s happening with stagflation and real estate prices? We’re entering a new era of inflation and stagnation, otherwise known as stagflation. The federal government has indicated that there will be no limit to money printing, quantitative easing and helicopter money. That spells higher prices, and eventually a devalued U.S. dollar. As the same time, we’re in the Greater Depression of 2020, which causes less demand and lower prices. Because GDP is being dramatically reduced by virus hysteria and lockdown, especially in major cities, many prices, urban home prices in particular, have downward pressure. The overall effect is slower growth with mostly higher prices for food and other staples, along with shortages and an economy that is out of wack. Most businesses shrink, or find it more difficult to expand. This is stagflation. Urban home prices go down, while suburban home prices either drop or stagnate, staying about the same price for longer than usual.
Q: Is there a place to live in a warehouse for rent los angeles? Where can I find warehouse living space for rent? A: There are warehouse lofts on the L.A. Loft Blog. Downtown Los Angeles Connection to Nashville Bombing
REAL ESTATE NEWS — DTLA has a personal relationship connection to the man who blew himself up in his motorhome. An L.A. entertainment executive is tied to the suspected Nashville Bomber. | Blog Video
Michelle Swing, a downtown L.A. resident and artist development director at AEG Presents, was reportedly given two houses by Anthony Quinn Warner, who has been identified as the perpetrator and victim of the Christmas morning bombing in Nashville, which devastated a city block. In that bombing incident, a motorhome was parked on the street and was making audio announcements of warnings to the public to evacuate the area. It also reportedly played a recording of Petula Clark’s 1965 pop hit, “Downtown.”
In January 2019, Warner reportedly gave Swing a $160,000 Nashville house via a quitclaim. The Antioch neighborhood is located 12 miles from downtown. Later, on November 25, Warner gave Swing another Antioch house worth $249,999.
It has not yet been revealed what the connection was between the 63-year-old Warner and Swing, who reportedly resides at the Gas Company Lofts in the Financial District, and has worked for AEG headquartered at LA LIVE in Downtown Los Angeles.
Swing has not yet responded to Loft Blog requests for comment. So far, we have found no other information linking Anthony Quinn Warner to Downtown Los Angeles. | MORE

As some older Americans are hit by deadly flu-like illnesses, we’re not seeing a significant increase in total deaths as a percentage of the population. The big impact is to be found in society and the economy. The future of real estate is in peril as young Americans now feel that all of their plans were ruined by the virus panic of 2020, according to The Guardian. The hysteria has led to economic collapse, worsening existing economic hardships, causing unemployment to triple for young adults – and has taken a serious toll on mental health. As the L.A. Loft Blog predicted, the overreaction is already causing far more damage than the disease. Sadly, the deadlier toll of panic shall get much worse, as millions defer basic health care, and millions of others have no idea what they’re doing right now, as they lose their jobs and health insurance. They’re maxing out credit cards to pay for bills and food.
The mainstream media, meddling politicians and social media monopolies continue to spread fear and hype, while failing to provide adequate mitigating disclosures about comorbidity, utterly failing to account for dire unintended consequences and massively censoring those who oppose hysteria and destructive lockdown. Doctors are paid extra to annotate assumptions that promote fear, fired for providing information that reduces hysteria. Hotels, restaurants, travel, tourism, entertainment and other service industries that hire a bulk of young adults are all but obliterated. In addition to today’s economic devastation, the long-term consequences of diminished education shall haunt the U.S. for decades.
While some have high hopes for the new year and new vaccines, U.S. economy will likely get worse before it gets better. Ned Davis Research says that the first quarter of 2021 will be rough.
As bloated governments take advantage of fear to grow bureaucracy uncontrollably, the best housing markets shall be capital areas like Sacramento and Washington D.C. Suburbs shall retain a robust property market while urban centers like Downtown Los Angeles shall continue to languish in exploding blight and falling home values. Eventually, even most suburbs shall also hit a wall, leading to some falling prices among single family homes. 2021 could be the year of overdue stock market capitulation and suburban surrender.
Commercial real estate is devastated nationwide, especially in Los Angeles. It can take years for commercial real estate to unwind, to continue to fall to its bottom. Shortages of food and household products evidence growing stagflation. Increasing government handouts are likely to land as direct increases to inflation.
While real estate continues to be a pillar of wealth and net worth in the American dream, the L.A. Loft Blog had the audacity to suggest that multiple property owners consider selling excess properties in order to put substantial money into Bitcoin. Those who took such a bold, innovative step saw their fortunes gain tremendously. We’re restating the suggestion once again today 🙂

There are no lofts in Palm Springs. It’s the land of single family homes, hotels and resorts that offer a popular refuge from the big city and cookie-cutter suburbs of Los Angeles. A few tiny homes speckle the desert, but they tend to heat up too fast in the extreme climate.
The first thing to be aware of when considering the purchase of a home in the Palm Springs area is the wind. There’s a reason why the area is home to many large windmills. It’s a wind tunnel. Fall, winter and spring bring very substantial windy conditions and wind storms that can blow like a sand blaster. When the wind blows from the southeast, a few times a year, Palm Springs gets a big whiff of the Salton Sea, with essence of sea salt and dead fish. Chances are that you already know that the area is a desert with summer temperatures sometimes rising above the 120 degrees Fahrenheit. That’s hot enough to burn your hand when you touch a door knob or car seat, and hot enough to hemorrhage cheap car tires.

Most homes in the area were built in waves, eras of time during the 1900s. This can be observed as bad plumbing problems in many of the houses built in the 1920s. When looking at homes of his period, always ask about electrical and plumbing issues. For 1970s construction, home buyers often find that they need to replace all of the windows.
This region is subject to flash floods. Some properties require a french drain to prevent briskly moving storm flows or accumulated ground water from intruding into the house.
Another unique facet of Palm Springs real estate is Native American ownership of much of the land — Indian land, which contains homes for sale on leased land, checkered throughout the area. Prospective home buyers must check to see if the land is leased land, or fee simple land, which means that the owner actually gets to own the land.
For those who would like to buy a house to rent out for retirement income, be aware that many landlords who attempt to make money from PS properties find that they do not make enough money. It’s not a cash cow for most. It takes knowledge and experience to make a profit from Palm Springs income properties and short term rental properties.
Some say that now is not a good time to buy a rental income property because short term tenants trash the house. They come in to party, and leave. Prospective home buyers looking at properties and neighborhoods to find that it is quiet on weekdays, but then the noisy parties erupt on weekends. Property damage and neighbor complaints are some reasons why renting out a house is not always extremely profitable.
Families in the area may find that resources are limited for parents with kids. Many of Palm Springs residents are mature singles and retirees with no kids.

For many homes that are flipped and staged, the new landscaping plants are mostly dead by August. Palm Springs is known for artists, who also come and go. They arrive here and don’t always do well. A few artists do quite well, such as the Savage Gallery. This most successful diversified artist and gallery has been selling artworks and furnishings to repeat clients, cranking out diligently with a uniquely aggressive energy and distinctive style. Savage stands out as the zestful, spirited sprite in a crowd of mostly retiring artists who are largely to be found loafing amid slackened motivation.
While many tourists and visitors enjoy shopping, swimming, hiking and biking, the long-term folks have other ideas. For many Palm Springs residents, sitting, relaxing and enjoying cocktails is the name of the game. Now, more than ever before, people go to Palm Springs to get away from it all.
Artist Brother Andy, who has flipped several homes in the area, warns about the extreme wind tunnel effect of the topography. High winds cannot be seen in photos, and the blowing comes out of nowhere, weeks or months after home buyers close escrow. Other nearby neighborhoods include Cathedral City, Rancho Mirage, Thousand Palms, Palm Desert, Indio, LaQuinta and Coachella.

Music Therapy Hit the Right Note for Nano
A love of mariachi helps a young patient battle a rare genetic condition.

He wears a large black sombrero with festive gold braiding, and the maraca in his hand doubles as his microphone. A guitarist strums and sings beside him as he tilts back his head and belts out the chorus:
Ay, yai, yai-YAAIIIIII
Canta y no llores
The tune is “Cielito Lindo,” a traditional Mexican folk song, and the performer is 6-year-old Adriano—a mariachi star-in-the-making better known to his many fans as simply “Nano.” And the concert venue? Nano’s bedside in the Bone Marrow Transplant (BMT) Unit at Children’s Hospital Los Angeles.
The BMT Unit has been the site of countless Nano concerts, thanks in large part to that guitarist singing along with him—Music Therapist Hana Cho, MT-BC.
“Hana is one reason he stayed alive in the hospital,” says Nano’s mom, Eliana. “She brought him something he loves: music. He lives for music. That’s just who he is.”
‘This can’t be happening’
Nano’s love affair with song—and especially mariachi—started at an even younger age. When he was a baby, his mom would soothe his tears by singing the chorus to “Cielito Lindo” (Pretty Darling).
“Every time he would cry, I would sing that to him: ‘Ay, yai, yai-yai, canta y no llores—sing and don’t cry,’” she translates. “Be happy! And that became his favorite song.”
The song has become a sort of anthem for Nano through the months he’s had to spend at Children’s Hospital Los Angeles. He was born with a rare genetic condition called Fanconi anemia, where mutations in certain genes slow the normal daily process of DNA repair in cells. This fragile DNA leads to bone marrow failure and other problems.
He was 15 months old when he was diagnosed at Children’s Hospital Los Angeles. “When the doctors told us, it just broke me,” says Eliana. “I was like, ‘This can’t be happening.’”
Some Fanconi anemia patients can live for years, even decades, before their bone marrow fails. But by the time Nano was 4, his blood cell counts were dropping too low, and random bruises began popping up on his body.
“As Nano grew, his marrow function worsened, and it got to the point where he was going to need blood transfusions,” explains Michael Pulsipher, MD, a hematologist and Head of Transplantation and Cellular Therapy in the Cancer and Blood Disease Institute at CHLA, and an expert in Fanconi anemia. “He really needed a bone marrow transplant.”
Bone marrow transplants in Fanconi anemia patients are highly specialized. Because of patients’ fragile DNA, doctors must use different and less-intense treatments, including lower-dose chemotherapy medicines.
Unfortunately, none of Nano’s family members—including his three siblings—were a suitable match. A search among unrelated donors also came up empty. Nano’s best option was an umbilical cord blood transplant from one of several national registries that bank cord blood that’s donated after babies are born.
“Cord blood is very rich in bone marrow stem cells,” Dr. Pulsipher says. “It allows us to do a mismatched bone marrow transplant that we couldn’t otherwise do.”
Born to perform
Nano’s transplant went well, but afterward, he developed graft-versus-host disease of the gut and skin, a common complication. He’s also suffered recurring bouts of pneumatosis, a painful condition where air gets in the lining of the intestinal walls.
Fanconi anemia patients heal very slowly. Nano’s hospital stay stretched longer.
“It seemed every time he would heal from one thing, he would get sick with another,” Eliana says. “It was just back to back.”
But throughout all these ups and downs, there was a constant bright spot: music. From the start of his hospital stay, Nano was visited weekly by Cho, the music therapist.
“Music therapy is not music lessons,” Cho explains. “It’s using music to support a child’s emotional and developmental health and well-being during the stress of hospitalization. We disguise therapeutic techniques through music.”
For Nano, though, it was all fun. He already had a “guitar” (really a small ukulele), and he was in awe of the many instruments Cho brought with her, including bongo drums, maracas, tambourines and more. At first, he and Cho sang standard preschooler songs like “Wheels on the Bus.” But it wasn’t long before he had another request: “Cielito Lindo.”
“He was like, ‘You don’t know that one?’” Cho says with a laugh, remembering the surprise on his face. “I said, ‘Well, I guess I’d better learn it!’”
Nano has several other Latin-inspired favorites, too—including “Un Poco Loco” (A Little Crazy) and “El Latido de Mi Corazón” (The Beat of My Heart), both from the Disney movie “Coco.” Cho learned them all, and soon, they were a regular musical duo on the BMT Unit.
“Tell everybody I’m going to do my songs,” Nano would instruct his nurse. “I’m doing a concert.”
Then he’d get into one of his authentic miniature mariachi suits–like his cream-colored suit with the fine gold embroidery, or his blue-and-gold vaquero-style outfit. His dad had brought the suits home after a trip to Mexico, and it was a 10-minute process for him to get ready—donning pants, boots, belt, vest, jacket and one of his signature sombreros.
With the nurses gathered in the hallway, Nano would start with his grito—a high-pitched whooping shout that is a traditional part of mariachi. And then the singing—and sometimes traditional folklorico dancing—would begin.
“At the end he’d take his hat off and bow,” Cho says. “He’s just a born performer.”
One of Nano’s nurses even sketched a picture of him in one of his mariachi outfits. And this past fall, Nano joined other CHLA patients in decorating a pair of cleats from Major League Baseball star Justin Turner, who hit a home run while wearing them.
On the cleats, Nano drew his favorite things: McDonald’s french fries, a sombrero and his signature lines: “Ay, yai, yai, yai … Canta y no llores.”
“Everybody knows him by that,” Eliana says. “Everybody knows who sings that song.”
Tears, joy and Iron Boy
Nano’s journey has not been all song and dance. There have been plenty of tears.
One tough moment came before his bone marrow transplant, when he first had a central line placed in his chest. Then 4, he was sobbing inconsolably, crying out, “‘Mom! Dad! Why did you let them give me an owie? Why? Why?’”
Those cries would break the heart of any parent. But his dad, Marco, had a sudden inspiration. He popped in the movie “Iron Man” and explained to Nano that the medicines he would be getting in his chest would give him the same super powers as the hero on the screen.
“You’re his partner,” he told Nano. “You’re Iron Boy.”
Nano’s tears quickly subsided, and his new nickname was born. His aunt even made him an Instagram page: “Iron Boy: My Road to Recovery.”
There were other tough times, too—like all the days in the hospital when he felt too sick to sing or dance or even lift his head from his pillow. On those days, when Cho arrived for music therapy, she would ask if he wanted her to leave.
His answer was always the same: Stay.
Sitting at his bedside with her guitar, she would sing soft lullabies while he rested. “He loves Hana,” Eliana says. “The connection they have through music—it’s not a connection you could have with anyone else.”
As you might expect, on the day Nano went home from the hospital, there was plenty of song and celebration.
For the occasion, he wore his best mariachi suit and a bright, camel-gold sombrero. In the BMT Unit, he gave a farewell grito. Outside, he and his parents were greeted by a mariachi band, as they sang along to “Cielito Lindo.”
Then, finally, he was home.
Stay strong, stay standing
Whenever Nano sees a TV commercial for Children’s Hospital Los Angeles, he turns to Eliana and says, “That’s my hospital, Mom.”
“Yes, baby,” she replies. “That is your hospital.”
His care team has spanned multiple areas of the hospital, including Hematology, Pulmonology, Intestinal Rehabilitation, Orthopaedics, Urology, Physical Therapy and more.
Nano’s family would also like to send a special thank you to the BMT nurses. “They’re our second family,” Eliana says.
Meanwhile, their journey continues. Recently, Nano was back in the hospital with another bout of pneumatosis, which quickly cleared. After slowly building up his tolerance to food again, he is now back home and doing well.
“Long-term, our hope is that we’ll be able to completely wean Nano off his immune suppression medicines, so he can live a more normal life,” Dr. Pulsipher says. “But we have to do that very slowly and carefully.”
Until then, “We’ve just got to stay strong and stay standing,” says his mom. “We’ve got to keep fighting.”
And of course, keep making music. And keep following those wise words from “Cielito Lindo”: Canta y no llores.
Sing and don’t cry.

Just like the weather seasons come and seasons go, so do the seasons of life. I’m sure you have noticed, as I have, the older I get the faster the seasons move by. These “seasons of life” go by so fast, my hope is that you enjoy each one or at least grow from each one. Yes. Some of life’s seasons will be HOT and others will be COLD, some high and some low. For them and their families, the Ground Hog seeing or not seeing its shadow is the furthest thing from their mind. Don’t get me wrong, these families long to see their kids out in the yard playing or riding their bikes – but for now, they are praying this Spring will be a season of healing.

A Preemie’s First Year: 12 Months of Worry
By Jeff Weinstock | A preemie bingo card is not one that any parent wants to have success filling up, but as darkly humorous recreation it served its purpose for Josie. She found it posted on an Instagram account by a fellow parent of a preterm baby and decided to play.
“It was like all the different things that can go wrong with your preemie,” she says.
Once she was done matching the squares to her son Lincoln’s many maladies, there was hardly an empty box left.
“I think we had like five bingos on there. Not everything that can go wrong, but most of it.”
Classified a micropreemie because he was born at 25 weeks—any earlier than 26 weeks earns the designation—and weighing only 2 pounds, Lincoln has faced a string of serious conditions. His need for expert, comprehensive treatment led Josie and her husband, Josey, to transfer Lincoln to Children’s Hospital Los Angeles in fall 2019, where he has been receiving care across several divisions, including Neurosurgery, Pulmonology, Cardiothoracic Surgery and Neurology.
“I feel like there’s been 12 months of worry,” Josey says. “I said to my mom early on, ‘I just want the worry to stop.’ And she said, ‘Oh, honey, I’ve been waiting 35 years for that.’”
Fixing the brain’s plumbing
Within hours of Lincoln’s birth on Sept. 5, 2019, the many perils associated with prematurity began to surface. He was found to have an intraventricular hemorrhage—bleeding in and around the brain’s ventricles, which contain the cerebrospinal fluid. The bleed can impede the flow of the fluid, causing a surplus to build up in the ventricles, a condition known as hydrocephalus. That overload of fluid puts pressure on the brain and can harm its development.
CHLA neurosurgeon Peter Chiarelli, MD, says he monitored Lincoln intently but didn’t act straightaway as Lincoln’s system was absorbing almost all the fluid his brain was manufacturing, so the excess took time to collect and show itself. “The presence of hydrocephalus was very subtle,” he says. “It wasn’t apparent until months were allowed to pass.”
At a visit in January, the indicators began to coalesce. An MRI showed that the surplus fluid was mounting, and in five weeks the growth of Lincoln’s head size had begun to accelerate, a sign his skull was trying to accommodate the increased pressure.
Plus, some anecdotal evidence weighed in. The family reported that Lincoln’s eyes would occasionally drift downward, more warning that the brain was under extra pressure. With that, the decision was made to move ahead with a procedure to divert the spinal fluid.
“Even though it was a lot to take in,” Josie says, “we knew the facts were clear. This is what he needs, this is what we’re going to do.”
In June, Dr. Chiarelli placed a drainage system, called a shunt, into the ventricles of the brain to siphon off the excess spinal fluid, which would then be emptied into the abdomen, where it could be reabsorbed. The pressure inside Lincoln’s brain returned to normal.
“Everything else in his body stayed completely in balance,” Dr. Chiarelli says. “It’s basically solving the brain’s plumbing problem.”
The shunt will stay put, continuing to drain excess fluid from the surface of the brain and grow as Lincoln grows. “That same shunt can support him at adult size. He’ll check in with his neurosurgeon once a year to make sure everything’s OK. If everything’s going well, it’s a quick visit.”
A sleep study disappoints
The brain bleed was unfortunately only one of two bleeding episodes Lincoln experienced. A day after his birth, he suffered a rare, frightening pulmonary hemorrhage that caused blood to stream out of his nose and mouth. Doctors were able to arrest the potentially fatal event, but it left a mark.
“We sort of knew it set us up for a longer-term pulmonary journey,” Josie says.
With preemies, intubation usually follows right after birth, to provide sufficient oxygen that their inexperienced lungs can’t deliver. At about a month old, the tube was removed from Lincoln’s airway and he began taking oxygen through a nasal cannula. Once needing it 24 hours a day, Lincoln now wears the cannula only when he sleeps.
His parents hoped that an overnight sleep study done in mid-September would show that Lincoln was able to stay oxygenated while asleep so he could stop wearing the nasal cannula altogether, but with 90 minutes remaining and his oxygen level falling, the cannula had to be reactivated.
“Given his degree of prematurity, it’s not entirely unexpected that he would continue to need oxygen,” Lincoln’s pulmonologist, Mark Selleck, MD, says. “We have children who need oxygen as late as 2 years. It’s disappointing for the parents, but it’s not surprising. He just needs to grow a bit more.”
Dr. Selleck says there will be some lasting impairment from the lungs’ underdevelopment, but it’s unlikely to be significant.
“Will he become an Olympian? Maybe not. Will he be able to play a good game of racquetball? Can he participate in sports and do all the things the rest of us do? Probably.”
A hole in the heart
A blood vessel called the ductus arteriosus caused more trouble for Lincoln. While babies are in the womb and drawing on the mother’s oxygen, the ductus arteriosus carries blood away from the inactive lungs, which are still developing, and sends it straight down to the aorta, optimizing fetal circulation.
After birth, the vessel should naturally seal up. Lincoln’s remained open, creating a doorway between the heart’s two main arteries—the pulmonary artery going out to the lungs and the aorta, which serves the body—where there should not be one, an abnormality called a patent ductus arteriosus (PDA). Blood meant to go out through the aorta to nourish the body flows into the lungs. Hence the condition’s informal name: a hole in the heart.
In preemies like Lincoln, cardiologist Patrick Sullivan, MD, explains, the ductus is more likely to stay open because, by its timetable, it’s still needed.
“It wasn’t meant to close until later in gestation,” he says. “They’re not supposed to be outside the body of their mom, right?”
The influx of blood forces a newborn’s lungs to work harder while depriving other organs of their portion.
“It places babies at greater risk for brain injuries, kidney injuries,” Dr. Sullivan says. “All of these organs can be compromised by having a ductus siphoning blood away from them.”
Weeks of medication failed to close Lincoln’s PDA, which began gaining in size, so Dr. Sullivan intervened. He had the benefit of a new device called a Piccolo Occluder specifically sized to close PDAs in premature babies. A catheter is entered through the groin and threaded up to the heart. Once at the blood vessel, the doctor releases the device—a plug that gets laid atop the vessel and caps it.
After a year of monitoring, if the plug is still functioning successfully, the issue is considered cured. Eventually, the once-troublemaking blood vessel goes away. “There aren’t really any problems that you anticipate developing after a year,” Dr. Sullivan says.
The post-op cardiology visits have been the least worrisome, Josey says. “They do an EKG and a little ultrasound, and then they say, ‘Yes, looks good.’ Those are our favorite appointments.”
Signs of seizure
In mid-August, Lincoln’s parents noticed he was repeatedly turning his head to the right.
“It seemed like something he couldn’t help,” Josey says. “He brought his hands together like he was trying to console himself, then he would come back to the center and look at me. But he would just keep turning his head to the right.”
That observation was enough for neurologist Andrew Ng, MD, to make a determination.
“A lot of times we don’t have the benefit of seeing the events, so we go off the history,” Dr. Ng says. “Based on the description, it sounded like a seizure. And if it sounds like a seizure, we treat it as a seizure.”
He started Lincoln on anti-seizure medication but had to switch it up a month later when Josey and Josie’s watchfulness led to a second abnormal finding. They saw that Lincoln had begun to let his head droop and was smiling less. Was it simply a negligible change in mood, or a sign something aberrant had occurred?
A six-hour video EEG, which allowed Dr. Ng to observe Lincoln’s behavior, unfortunately proved it was the latter. This time the diagnosis was infantile spasms, an epilepsy syndrome associated with early childhood.
Dr. Ng replaced the medication he had prescribed to deal with Lincoln’s initial seizure with one that he says can handle both.
This need to keep a trained eye on Lincoln’s demeanor can put his parents in a hard position, trying to discern an insignificant gesture from a notable change in behavior. Consider the puzzle they face with drowsiness, which can be an alert for seizures or a malfunctioning shunt.
“You don’t want to always be thinking that, oh, he’s drowsy, therefore there’s some terrible thing happening, because people get drowsy,” Josey says.
“Yeah,” Josie follows up, “maybe he just ate a big breakfast.”
Dr. Ng says it’s something they will become more attuned to as they continue to live with the specter of seizures. “It’s not like the skin, where if you have a cut it heals up. We’re born with all the neurons we’ll ever have. Unfortunately, this is not something the brain can outgrow.”
Prospects and promises
Shortly outside a year since he was brought to CHLA, Lincoln’s care now focuses on his development. He’s working with therapists on his motor skills, speaking and cognitive function, all of which are lagging. It’s unknown how much catching up he can do, and to what extent the effects of prematurity, particularly the initial brain bleed, will obstruct him.
“We don’t know what that might mean for him going forward,” Josie says. “We’re going to keep pushing him toward all the milestones, and we’ll figure out what his prospects are as we go along.”
The only hard cap placed on his potential precludes him from ever becoming an astronaut, a consequence of the shunt. “If that’s his limitation, to stay on Earth, then I suppose it could be worse,” Josie says.
There have been victories, too, recorded dutifully by his parents: feeding himself with a spoon, sitting up for longer stretches of time, his first utterances.
“For the first time, he’s almost on the growth chart,” Josie says. “He’s in the 1st percentile, which for us, we’re like, ‘Yeah!’”
She recalls the first time she got to see Lincoln, after he had been rushed off to intensive care right after birth. She could talk to but not touch him.
“He didn’t look like a normal baby. He was hooked up to so many monitors, and he had the breathing tube down his throat. And he had a little mask over his eyes because he was having the blue-light treatment for his skin. I saw him and I cried. And I said something like, ‘I love you and I’m your mom.’ Then I paused and I said, ‘I don’t really know what to say yet, but I’ll get better at it.’ And then I told him about a lot of things. I told him, and I remember I needed to tell him in that moment, I said, ‘It’s not supposed to be like this, and it will be better. Inflation: Panic Prices and Real Estate
Pandemic price increases abound on Amazon.com, according to Financial Times.
Essential items see sustained significant price increases on Amazon in 2021, with some products jumping up many times their original price. Analysis by the US Public Interest Research Group studied 750 essential items sold on Amazon’s marketplace, comparing their pre-panic prices to what customers paid for them at the end of 2020.
Of 750 items, the prices of 409 increased by more than 20%, while 136 had more than doubled. Patio heaters, suddenly a must-have during winter lockdowns, saw the most significant percentage increase. One model skyrocketed from $150 to $699 — a 366% jump. | Blog Video
Home Prices Plateau
Despite massive federal spending, money printing and quantitative easing, some experts expect home prices to see a smaller rise this year than in 2020.
Southern California home prices and sales rose in December from a year earlier, continuing a so-called “pandemic housing boom” driven by rock-bottom mortgage rates and people needing more space to cope with lockdown.
The 6-county region’s median sales price jumped 10.1% from December 2019 to $600,000 last month, according to data released Friday from real estate firm DQNews. Sales rose 29% from a year earlier.
Real estate agents and other housing experts say the pandemic had supercharged the market. People with higher incomes who are most likely to buy a home in the first place have been relatively unscathed by the economic downturn. And some are looking for more space as some workplaces remain closed. Mortgage interest rates dropped to record lows, in part because of a Federal Reserve policy designed to stimulate the economy. Government -controlled mortgage company Freddie Mac reported the average rate for a 30-year fixed mortgage was 2.77% this week, down from 3.6% a year earlier. The drop has lured more buyers into a market in which they’ve found few homes for sale, prompting bidding wars on the most attractive homes.
Like gold, stocks and bitcoin, real estate provides a hedge against inflation. In Los Angeles County, the median price rose 11.4% from a year earlier to $700,000, while sales climbed 26%.
The upswing in the for-sale market is in stark contrast to the rental market, where struggling tenants have disproportionately been hit by job losses. The contrast shows how the coronavirus panic has exacerbated economic disparities in California, and in the U.S. at large.
Many low-income renters are behind on rent and worry they’ll eventually face eviction during a pandemic that has killed more than 400,000 Americans. The average rent in Los Angeles has also fallen as vacancies rise, in part because some higher-income renters are choosing to buy for the first time.
Stagflation, the collapsing economy with higher prices, best explains the crazy combination of up, down and stagnant prices. While it is true that some suburban neighorbhood prices went up in 2020 because families and office workers were locked down, forced to stay home more, the prices increases reflect a temporary need for more suburban style safety and more square footage due to temporary overuse of residences, not because of a long-term increase in value. Based on true accounts from actual real estate professionals in Los Angles, reports of a “pandemic real estate boom” are as overblown as other pro-pandemic propaganda that we are being forced fed by politicians and biased media. Most home values are stagnating or falling nationwide. Most urban home prices have been tumbling for several years. Commercial real estate has fallen off a cliff, as investors like Ben Mallah sell loser hotels in favor of safer properties anchored by major national brands.

Lunatic Policies Continue to Chase Wealth Out of the State as the Tax Base Dwindles
REAL ESTATE NEWS (LOS ANGELES, CA) — People are leaving in droves. The State of California has its head buried deeply in the sand as looming deficits soar to unimaginable heights. The state’s rapidly failing policies of excessive taxes, soaking the rich, stealing from the middle class and expanding homeless tent cities continues to discourage the biggest and smartest companies in the state. They’re leaving at an accelerating pace.
Yahoo Fake News says the exodus is accelerated by coronavirus. This is a false narrative, as viruses have proven to operate basically the same in all states, spreading and dissipating without much regard to politics. Doctors are being censored when attempting to communicate the truth about the number of deaths being normal, like a typical flu season. It is not a virus, but panic, hysteria, exaggeration and overreaction that have accelerated the already brisk movement of people, wealth and brain power out of the State of California. According to tens of thousands of medical practitioners, heavy-handed lockdown mandated by state politicians has damaged health, wealth and happiness of citizens. Cautioning against fear and unwise practice, a growing number of doctors call it the “greatest hoax ever perpetrated on an unsuspecting public”. Californians will eventually rely on their own eyes to see that fake news and corrupt politicians are wrong.
This week, Oracle announced that it is moving its headquarters to Texas. The fake news blames the virus, but The California Exodus has been happening for years, and was already hitting critical mass before the virus hysteria arrived. Oracle is not moving to Texas to get away from a pandemic. With 135,000 employees, the database giant is moving away for the same reason as Tesla, Hewlett Packard and so many others: California’s extreme economic policies, over-regulation, high cost of living, growing crime and the trend toward telecommuting. Wealthy, smart people and their powerful companies have no reason to stay where they are not appreciated. When they are over-taxed, over-regulated, stifled and disrespected, they pick up and move to another state, or another country.
The State of California has shortsightedly ignored economic, as well as physical and psychological effects of over-taxation, over-regulation and increasing crime. In 2020, the myopia exploded into full hysterics by mainstream lamestream fake news media, politicians and irrational support by a significant percentage of a paralyzed population under massive censorship and propaganda by unlawful social media monopolies like Facebook and Youtube.
It’s time to stop believing the virus hype, and time to get concerned about being the last souls left in a lifeless state dominated by blight, refuse and tent encampments. Unless these unwise trends stop quickly, California may not recover any time soon. The wealthy and middle class will continue to leave the state, leaving mostly poverty and blight in the once golden state. THE TWO MOST PROFITABLE THINGS
Let’s start with what has worked the best for me over the past 10 years. That has been two things I have done: One is real estate as a licensed real estate agent, real estate salesperson, licensed real estate broker and also a credential as a mortgage loan originator — and then, the second thing, which is equal in importance as the Bitcoin blockchain cryptocurrency and profitably mining — which comes from learning about how this all works.
How it all works together, thats what were actually doing now. We’re creating new ways to connect those two big topics of real estate and blockchain. They can work together so that they amplify each other. Thats where the billion-dollar plan comes from. That has been The Secret of My Success. Without doing that, I dont know what I would be doing now. Perhaps serving Slurpees at 7-Eleven, or working at McDonalds or, with my experience, I would be probably more like working as a marketing director of marketing for some company or something like that, or I could just be doing sales, using my experience, or I could be doing work with blockchain — probably sales and marketing for a blockchain company. In fact, I’ve initiated communication with the people at Bitcoin Suisse in Switzerland, and working on my Swiss German.
Let’s give you the basic plan of the billion dollars in real estate starting with little or no money. First of all, there are two trains of thought when we’re talking about success. One is the fact that some people want an overnight success. Other people, however, understand that the strong, lasting foundation of long-term success is the kind that takes a long time to develop. That’s what we’re really looking at: The Best of Both Worlds, which is the fastest way to generate revenue — by taking advantage of real estate and blockchain. It’s also the most durable, strong foundation — the longest lasting. I’ve seen now a lot of both: quick money that disappears quickly; and also, very long-term durable success.
For the most part, the quick overnight success give the most fleeting results that disappear the fastest. The success that takes the longest to build up, however, is the strongest foundation and the longest-lasting success. That is what we are going to be focused on: Merging of Two Worlds together.
Let’s start with a little bit of background first. Let’s talk about some failures on my journey as an entrepreneur for the last 40 years or more. You can call a lot of things failures. Let’s do that. I’ve done things from doing wholesaling of imported products, doing internet video and stuff like that. We can call that successful, but I’m going to call them failures right now just to make a point, because that’s what some people might call them if they did not make a million dollars or a billion dollars. Those things that I accomplished earlier in my career generally made just sort of pocket change, comparatively. Online video that I did generated thousands of dollars a month, but not not enough profit to be a full income. So that was successful, but not as successful as my goals required it to be. And then there was those products that really only made a few dollars here and there by the wholesale products. That was a success in that it would generate a profit, but did not make enough profit to live on. They offered no vision for a long-term success, so they were not on the level of where we’d want it to be, so those are the examples of minor successes that we’re going to label as failures for the sake of this discussion.

I had a career in the U.S. Air Force. I would maybe not call it a career, as it was just a four- year term. It was a success in that I was promoted normally, on time, and was offered the opportunity to stay in the military, but I got out to get into the sign business. That again also was something you could call a failure or success because we generated enough money to live off of it for several years, but never enough to feel that it was going to be progressing financially to where it needed to be.
Let’s fast forward to about 10 years ago. When I started doing real estate in downtown Los Angeles, that immediately generated more income than what most jobs that I had done. So that was good. And then it got to the point in 2012 to where it generated far more money than what I had made at any other job or career that I had done. 2012/2013 was the period that made enough extra money so that I bought Bitcoin — quite a bit of Bitcoin. It had just crashed and just made the news because it had shot up like crazy, and then crashed quite a bit. That that thats how I noticed. It caught my attention. That’s how I first heard about it: on the news. They said Bitcoin shot up and then crashed. That caused me to look it up on Google and so forth. But I didn’t know how to buy Bitcoin, so I just got on Facebook to ask some Facebook friends, then one of them said, “Go to localbitcoins.com and buy some. I did. I bought some Bitcoin on localbitcoins.com, then I rewarded my friend by giving him a Bitcoin. That Bitcoin, one Bitcoin, is now worth around $15,000 to $20,000 so, thats a pretty good thank you gift.
I ended up giving Bitcoins to numerous other people, too. If theres one thing that I want to brag about, it’s that I’ve given more Bitcoins to more people than what most people in the world will ever own in their entire lives. So I’m thankful to be able to help people in that way. Because that really is the best blessing in life — to be able to to give blessings to other people. It’s all about helping other people, and that ultimately ends up helping ourselves the most — when we focus on improving and increasing the power at which we’re able to help other people. Thats what it’s all about. That’s how free enterprise works. That’s how giving works. That’s how financial success works. Giving is also how spiritual success works: success as in happiness and satisfaction in life and beyond.
REAL ESTATE AND BLOCKCHAIN ARE ONE
So that’s how we got started, and that’s how we got into real estate and blockchain. It took hard work, determination, knowledge, experience and failure to forge a strong, durable success. It took teamwork. It took years for a dynamic team, working with other teams and individuals: students, masters, entrepreneurs, investors, technicians, engineers, vendors, infrastructure, marketers, salespersons, customers, clients and partners. A Pair of Daring Heart Procedures Save Preemie Alex’s Life
Combining expertise with ingenuity, a team of cardiovascular specialists fixes a rare birth defect in a premature baby.

Sarah Badran, MD, knows her audience. “Let me know if I lose you, all right?” she says.
An interventional cardiologist at Children’s Hospital Los Angeles, Dr. Badran starts into a plainspoken description of blood circulation that she keeps for the layperson, whether an anxious parent or an easily baffled writer.
She calls on it here to illustrate the birth defect that led her to perform a virtually unprecedented cardiac catheterization procedure on tiny Alex, born Oct. 26, 2020, at 26 weeks with his lungs unattached to his heart, creating a disrupted blood flow that threatened his life.
She begins with a familiar association to explain how one’s blood moves oxygen from the lungs out to the body: “The blood is like an Uber driver,” Dr. Badran says. By the time she finishes, Alex’s disorder is understood and ready for retelling.
In normal circulation, the lungs load the blood with oxygen and send it through the pulmonary veins to the heart’s left atrium, and then out to the body’s organs through the aorta. Then depleted of oxygen, the blood returns to the right side of the heart before getting pumped back into the lungs, and the whole circuit repeats. Before birth, this exchange is formed anatomically, as the pulmonary veins become anchored to the back of the left side of the heart.
In rare instances—1 in 10,000 births—that attachment doesn’t develop. The pulmonary veins “just get lost,” Dr. Badran says, draining the blood out elsewhere within the body and leaving it without any road back to the heart, a birth defect called total anomalous pulmonary venous return (TAPVR).
“What generally happens is, you have a beautiful little baby, they’re born, and then they turn completely blue. That’s usually a big, big emergency.”
To the liver and back
The condition is not easy to see on a prenatal scan because fetal circulation doesn’t involve the still-developing lungs, but instead draws oxygen from the mother’s placenta, so blood flow tends to appear fine.
Alex’s abnormality wasn’t discovered until a week after he was born, when his breath began to grow short. An echocardiogram revealed he had TAPVR and that his pulmonary veins were depositing blood into his liver. The diagnosis got him transferred to Children’s Hospital Los Angeles.
Alex’s blood, however, was managing to return to his heart through a blood vessel located in the liver called the ductus venosus. The same vessel is employed in utero, though that’s by nature’s design, as it passes oxygenated blood to the heart after receiving it from the umbilical vein, which extends out of the placenta. Shortly after birth, that ductus venosus typically closes off and a new breathing apparatus, led by the lungs, takes over.
The irregularities caused by prematurity may have saved Alex, as his ductus stayed partially open, allowing blood to stream through it and reach his heart. Once there, the blood would pass from the right to left chamber through a hole called the patent foramen ovale, and then, freshly oxygenated, flow out to the body.
“It was a wackadoodle circulation,” Dr. Badran says, “but that’s how he was staying alive.”
The one chance to save him
That passage through the ductus venosus, however, was on its way to sealing shut, so some intervention had to be made. Dr. Badran and her colleague in CHLA’s Heart Institute, Neil Patel, MD, discussed next steps with cardiothoracic surgeon John Cleveland, MD, who would do the open-heart surgery that Alex needed to fix his congenital defect. They agreed that the operation was too risky for a baby so unstable.
The safer option was to wait until Alex developed more, as close to full term as they could get him, before Dr. Cleveland would operate. In the interim, the plan was for Dr. Badran and Dr. Patel to put a stent into the ductus venosus to keep it open so it would continue to convey blood to Alex’s heart.
That decision meant Dr. Badran and Dr. Patel were faced with trying something that, according to Dr. Cleveland, had only been done a handful of times worldwide, and never at Children’s Hospital: stenting a ductus venosus in a premature baby. The procedure is so unusual because TAPVR, as uncommon as it is across all births, is hardly ever reported in preemies.
Dr. Badran and Dr. Patel were unfazed. They were confident they could apply fundamental techniques and their accumulated expertise to an entirely new predicament.
“The thing about pediatric interventional cardiology is that it’s always like that,” Dr. Badran says. “Everything is so rare we’re always kind of engineering our way through these types of situations. I know everything I need to know about premature babies, and I teach embryology in med school. I know everything there is to know about the ductus venosus system, blood flow, and what the vessels are like, how they’re shaped, and why they close and why they open.
“The only chance to save Alex was to try to stent this open, which would not be fixing the problem. It would just basically be making it like he hadn’t been born yet.”
The day after the determination was made to try to install a stent, Alex’s blood pressure and oxygen levels began to fall, signs the ductus venosus was closing. The doctors were compelled to move Alex, only 17 days old, into the catheterization lab. Dr. Badran recounts the delicacy of the procedure—and exults over its success.
“To take this tiny little person,” she says, “and put an IV in his neck vein and try to find this threadlike thing, thinner than a wire—we almost couldn’t find it—then stretch it with a balloon and put a stent in—it was a technical tour de force. It worked, and it stabilized him.”
Restoring the anatomy
He was stabilized but not safe, and would spend the next several weeks watched by neonatologist Jennifer Shepherd, MD, in the Steven & Alexandra Cohen Foundation Newborn and Infant Critical Care Unit. “When I heard we were getting a baby who was 26 weeks with obstructed infradiaphragmatic TAPVR,” she says, “my first thought was, how are we going to keep him alive? It just was unheard of.”
As they worked to maintain Alex’s circulation until he reached an age and a weight suitable for surgery, Dr. Shepherd and her team had to respond to any unfavorable movement in his oxygen levels or blood tests. “We had a very low threshold to get echocardiograms to make sure that stent was still open.”
Alex made it to 37 weeks in gestational age, but not without needing more time in the cath lab to keep the stented area from closing. At 37 weeks, imaging showed the ductus venosus was again narrowing, so on Jan. 5, Dr. Cleveland brought Alex to the operating room to establish a connection between his lungs and heart. Though Dr. Cleveland had done the procedure often, Alex’s size—still only 4 pounds—added to the risk.
“Repair of total anomalous pulmonary venous return is a routine operation; we probably see six or seven cases a year,” he says. “There’s just less room for error when children are that small.”
With Alex on the bypass machine and his body cooled to about 65 degrees Fahrenheit, Dr. Cleveland removed his heart from his chest and began the repair. He tied off what he calls the “anomalous pathway,” the ductus venosus that had kept Alex’s blood flowing, then sewed the pulmonary veins to the back of the left atrium, where they should have originally attached. He set Alex’s heart back into his chest, warmed his body up and returned him to intensive care.
“Everything went well,” Dr. Cleveland says. “The goal of the operation was to restore normal anatomy, which we did.”
Follow-up scans have all looked good. With Dr. Patel staying on as Alex’s cardiologist, Dr. Cleveland expects his work has ended. “He’ll get echocardiograms because that’s part of the deal, but this should be the only heart operation he needs for this condition.”
It’s not often that open-heart surgery on a newborn is the postscript, but while it culminated Alex’s heart repair, from all accounts what set it apart was the opening act. Stenting the ductus venosus on a preemie that small was not merely delicate but untried.
“From my review of the literature, nobody else in California had done it,” Dr. Cleveland says. “I think it was a California first.”
While exhilarated by the achievement, Dr. Badran saves her strongest, lengthiest accolades for the efforts of the anesthesiologists and the neonatologists and other support staff, “the village that it took,” she says, to keep Alex intact so she and Dr. Patel could do their work.
“To transport such a tiny baby, that sick, get him safely on the cath lab table without dislodging the breathing tube, and give him the right medications, and the way the nurses and the techs all coordinated, and hospital maintenance heating up the cath lab to 82 degrees to make sure he wouldn’t get cold—they somehow managed to keep him alive while we found that vein.
“We had stented tiny things on lots of babies, getting into small little blood vessels and opening them, but the amount of cooperation and collaboration between all the divisions, it was just a beautiful orchestration of teamwork.”
